Hypothyroid and THEN Hashimotos? Is that right?

Hello lovely people. I hope everyone is holding up in these intense times 🙏🏾💜

I was diagnosed hypo over 5yrs ago but had a terrible for a few years until I paid a fortune for many private tests and they found that I was u able to convert. Once I started taking t3 that helped. Around mid March I believe I caught a mild version of Covid. We were all in lockdown and no testing was done so I can’t be sure but in any case I spent several months shielding which would have probably affected us all in one way or another. My low energy etc came back but I was overall ok ( not great, just ok)

I had my annual private broad range tests this summer and they were abit off and revealed very high antibodies- the first time this has ever happened. Can anyone please shed a light on what they mean? My GP won’t accept private tests but won’t do as extensive tests as I can do privately which is crazy... I am on 100mcg Levo and 30mcg T3 and gluten free for a couple of years. THE RESULTS IN BRACKETS ARE FROM LAST YEAR FOR COMPARISON

TSH: 0 ( 0.08) 0.40-4.50

FT3: 6.56 ( 4.09) 3.07-6.76

FT4: 16.17 ( 11.30) 11.97-22.01

T3 total: 1.99 ( 1.24) 1.17-2.79

T4 total: 69.4 (52.6) 57.92-154.44

T3 uptake: 27.20 (32.89) 22–37

TPO abs: 11 (16) 0-34

Thyroglobulin abs: 331 ( 13) 0-115

RT3: 0.68 ( 0.51) 0.28-0.95

TT3:RT3 9.65 (n/a) 2-20

Vit D: 148.26 ( 174.74) 75-250

Ferritin: 105.10 ( 129.60) 30-400

Folate serum: 18.57 (45.32) 12.46-61.18

% transferrin saturation: 58.5(55.9) 20-50

I’m so confused as to what is happening and how I can help myself. I eat well, still no energy to do the sports I used to love, generally feeling ‘off’ and have terrible constipation in spite of clean diet and lots of water/ hot water and lemon etc. I supplement under the guidance of the london clinic of nutrition who have been amazing. I know I have low stomach acid but I’m really at my wits end and really grateful for any advice you can shed on the above and what I can do to try and get my life back!

Should I tweak my T4/T3 combo? Do I have hashis and was it potentially triggered by covid if I did have the virus?

How do you normally take your T3 - as a single dose or split into 2 or 3 doses?

Do you always get same brand of levothyroxine and same brand of T3

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)

If/when also on T3, make sure to take last third or quarter of daily dose 8-12 hours prior to test, even if this means adjusting time or splitting of dose day before test

Is this how you do your tests?

All your vitamins have dropped a bit

Ft4 and total T4 both look a little low, though not as low as last year

Ft3 and total T3 are on higher side

Perhaps consider reducing T3 by 5mcg and small increase in levothyroxine

Thank you so much! Yes I do tests to the letter as per your instructions here. And I take all my meds in the morning and don’t normally eat anything for an hour or two. I find that taking them all at the same time really help as I’m ‘punch drunk’ when I wake up. I was more concerned about the thyroglobulin antibodies though. They were 11 & 13 the previous two years and now over 300 and well over the range. What do you think could have happened? And does this mean I now have hashis? Should I get a scan on my thyroid? X

It might or might not be significant....

High TG antibodies can make you feel more unwell
