Test Results Advice Please

Hi, these are my latest vitamin test results

Vit B12 613 (211-911)

Folate 10.16 (>12 normal <7.63 deficient)

Vit D 24.73 (<20 deficiency, 20-30 insufficiency, 30-100 sufficient)

My ferritin wasn’t tested because the doctor said my haemoglobin was good 14.1 (12-15)

I’ve been given 50,000 Vit d tablets one a week for 13 weeks then retest and 1mg of folic acid daily for 13 weeks and retest.

Please advise if this is the best way forward? Do I need any Vit b12? Thank you for reading my post.

Is the unit of measurement nmol/L or ng/ml?

1mg of folic acid daily for 13 weeks and retest.

That would seem reasonable. I think where deficiency is present it's 5mg folic acid, you don't come into the deficiency category but it's below the normal level so 1mg daily and retest in 13 weeks would seem to be OK.

You haven't given the unit of measurement - it could be pmol/L, ng/L or pg/ml (the latter two being the same).

According to an extract from the book, "Could it be B12?" by Sally M. Pacholok:

"We believe that the 'normal' serum B12 threshold needs to be raised from 200 pg/ml to at least 450 pg/ml because deficiencies begin to appear in the cerebrospinal fluid below 550".

"For brain and nervous system health and prevention of disease in older adults, serum B12 levels should be maintained near or above 1000 pg/ml."

So your level would appear to be OK but could be better.

I'm thinking you're not in England or Wales where the unit of measurement is usually nmol/L and for your level of 24.73ng/ml (61.82nmol/L) you wouldn't be prescribed anything let alone loading doses. Drs only prescribe to bring level up to 50nmol/L then we're on our own because that then reaches the "sufficient" category.

So your loading doses are 50,000 x 13 = 650,000iu D3. The NICE Clinical Knowledge Summary for England suggests loading doses of 300,000iu in total so you are extremely lucky.

The Vit D Council recommends a level of 50ng/ml (125nmol/L) and the Vit D Society recommends a level of 40-60ng/ml (100-150nmol/L).

You will have to see what your level is when retested and if you've reached the recommended level then go onto a maintenance dose to keep it there. Post your new result when you have it for further guidance.

Your GP maybe won't know, because they're not taught much about nutrients, but there are important cofactors needed when taking D3 as recommended by the Vit D Council

D3 aids absorption of calcium from food and K2-MK7 directs the calcium to bones and teeth where it is needed and away from arteries and soft tissues where it can be deposited and cause problems such as hardening of the arteries, kidney stones, etc.

D3 and K2 are fat soluble so should be taken with the fattiest meal of the day, D3 four hours away from thyroid meds if taking tablets/capsules/softgels, no necessity if using an oral spray

Magnesium helps D3 to work. We need Magnesium so that the body utilises D3, it's required to convert Vit D into it's active form. So it's important we ensure we take magnesium when supplementing with D3.

Magnesium comes in different forms, check to see which would suit you best and as it's calming it's best taken in the evening, four hours away from thyroid meds if taking tablets/capsules, no necessity if using topical forms of magnesium.

Discussed above. If you want to improve your level then you could take a good B Complex containing methylcobalamin and methylfolate. Consider Thorne Basic B or Igennus Super B.

Discussed above. Your 13 weeks of folic acid should bring you up to the normal level, see what it is when retested. If you have reached the normal level then then one of the B Complex supplements mentioned above contains enough methylfolate to maintain your level.

Oh, OK. Well, when looking for a B Complex look for the words "Bioactive" or "Bioavailable" and methylcobalamin (not cyanocobalamin) and methylfolate (not folic acid). Those will be the best quality supplements.
