Hi there,

I have been a member here for a few months now. I find the comments here very helpful and reassuring. So thank you.

I was diagnosed with auto-immune hypothyroidism several months ago. I began the standard treatment protocol starting with 50mcg levothyroxine and rising to 125mcg levothyroxine. I felt OK for a period, and then symptoms returned. Under the direction of a private endo I am now on combination treatment of 10mcg liothyronine and 100mcg levothyroxine. I feel like things are improving and on the right track but am not anywhere near 100% restoration of well being.

I have a pretty good diet and lifestyle. Have mostly cut out gluten and dairy. I have no idea why the antibodies appear to be so high?

The main symptoms I battle with are low mood, fatigue and brain fog. I also have persistent ringing in my ears and a brain throbbing sensation.

Your antibodies are high because you have Hashi's. And, by the look of it, you've not long had a Hashi's 'hyper' swing - coming back down, now. That's all the antibodies signify. Very little to do with diet and lifestyle - although some people claim that a 100% gluten-free diet reduces antibodies, I've never seen adequate proof of that. But, even if it did reduce the antibodies, you would still have Hashi's, because the antibodies are not the disease. But, 'mostly' cutting out gluten won't do anything.

Your FT4 and FT3 are slightly high at the moment, and that could be why you feel ill. But, doubtless they will eventually go down again.

On the other hand, your B12 and folate are on the low-side, and could be causing some of your symptoms. :)

I don't fully understand what a Hashi's 'hyper' swing is. I get that the disease will continue to produce anti-bodies, but the anti-bodies in themselves do not cause symptoms (although some say they do and gluten-free may help, which makes sense to me). The symptoms are caused by inadequate hormone levels. However the manual replacement of T4 and T3 via medication restores the correct hormone balance and keeps the symptoms at bay. So what is a hyper swing and why does it cause symptoms?

What would you recommend in terms of current medication? Stay the same, different combo, higher/lower dose?

Do you have any advice about targeting specific symptoms such as brain fog and ears ringing? I seem to be nearing the optimal level of medication (at least as far as TF3 and FT3 go) but these particular symptoms persist.

Can you recommend a particular supplement so I can boost vitamins and minerals to the upper range?

I see you're very active on the forum and provide many people with a great deal of advice. So thanks again.

A 'hyper' swing happens when the immune system attacks the thyroid and the dying cells dump their stock of hormone into the blood. This causes levels of T4 and T3 to rise sharply, and the TSH therefore drops.

The antibodies are highest just after an attack, because they come in to clean up the mess left behind. During an attack traves of TPO and Tg leak into the blood, where it shouldn't be, so the TPO/Tg antibodies envelop it and take it away to dispose of it.

As your antibodies are high on those results, along with high FT3/4, I assume that you had an attack not long ago. I could be wrong, but it looks like it. I would suggest that you stay on the same dose of thyroid hormone replacement, at least for the time-being, and see how it goes. When do you plan to get your next test?

Your B12 is so low, I think it would be advisable to get tested for Pernicious Anemia before supplementing anything. PA is autoimmune, as is Hashi's, and when you have one autoimmune disease, you are likely to develop another. So, insist on getting that tested. If you have PA, you will need B12 injections for life.

Brain fog should improve with time, but the brain does also need adequate levels of B12, so do get that sorted out. Tinnitus is more difficult. For some people it goes away when both hormone and nutrients are optimal. For me, it never has.

If, by that, you mean a multi-vit, then no. I would never recommend a multi-vit, too many things wrong with them.

Vitamins and minerals need to be tested and supplemented individually. Your ferritin is good, anyway. I can't see a result for vit D. If you get that tested, that would give us more information about how you should supplement. However, you could try taking vit C, magnesium and zinc to see if they help. But, don't start them all at once. Leave about two weeks in between starting each one. :)
