Supplements I take are 150 mg of levothyroxine a day b12 and folic acidHad private thyroid ultravit with medicheaks here are my results Thyroid stimulating 9.62 / range 0.27-4.20 Free thyroxine 14.300/ range 12.00-22.00 Free t3 4.07 / range3.10-6.80. Thyroglobulin antibody 72.300 / range 0.00-115.00 thyroid peroxidase antibodies 378 / range 0.00-34.00 vitamin d 50.1 /range 50.00-200.00 crp high sensitivity 14.05 range 0.00-5.00 has b12 folates and ferritin done which havnt put results and ranges because all ok

Are you taking any thyroid meds, if not you should be with TSH of 9.62? But if you live in GB your doctor might want it to reach10! You have Hashimoto's disease with raised TPO ( peroxidase) thyroid autoimmune antibodies. The antibodies intermittently attack your thyroid gland mistakenly believing it to be 'alien', the dead cells along with thyroid hormones are dumped into the blood stream resulting in periods of peaks of hormone. Some are able to limit these antibodies by trying a gluten free diet, so these peaks or 'flares' are reduced. Another characteristic of Hashimoto's is poor gut absorption resulting in low mineral and vitamin levels whether from food/drink or supplements- thus it is worth checking VitD, B12, folate and ferritin levels as you have done in part with vit D . For good thyroid health , and conversion of T4 to the active T3 our cells need , these levels need to be optimum, in the upper levels of ranges. Your Vit D is low at 50.1, it needs to be 100-150. The CRP figure showing inflammation is raised; and you have no folate/ferritin results. Have you shown them to your Gp: need to start a 50 mcg dose of levothyroxin, and have bloods tested in 6 weeks ( probably only TSH on NHS)- upping dose if necessary to get TSH to 1 or under? Ask your doctor to test Vit/ mins too; he/she won't offer help for Hashimoto's, but they might recognise it is destroying your thyroid!

Leave a copy of your results in the surgery for your doctor to look at.

Tell him/her you've had Advice from Healthunlocked Thyroiduk.org.uk (the NHS Choice for help/advice) and you should start on 50mcg of levothyroxine. Please leave a prescription for collection.

I am shocked that you are on 150mcg therefore there's something not quite right in that your TSH is nearly 10 whereas it should be 1 or lower.

It would seem you are a poor converter of levothyroxine into the only Active thyroid hormone T3 so you should really be given a T4/T3 combination:-

Thyroid stimulating 9.62 (far too high) / range 0.27-4.20

Free thyroxine 14.300/ range 12.00-22.00 - should be towards the upper part of the range.

Free t3 4.07 / range3.10-6.80. - should be nearer 6.

Thyroglobulin antibody 72.300 / range 0.00-115.00

thyroid peroxidase antibodies 378 / range 0.00-34.00

(antibodies high which means you have Hashimoto's and going gluten-free can help prevent the attack of the antibodies on your thyroid gland.

vitamin d 50.1 /range 50.00-200.00 - this is bottom of the range.

Also, you could do with a slight increase in your Levo as the aim of a treated hypo patient generally is for TSH to be 1 or below or wherever it needs to be for FT4 and FT3 to be in the upper part of their respective reference ranges when on Levo if that is where you feel well. It looks as though you don't convert T4 to T3 very well, going by your FT4/FT3 results. Supplementing with selenium l-selenomethionine 200mcg daily can help with Hashi's as it can help reduce antibodies, but it also helps T4:T3 conversion.

You need an increase. When was your last bloods with doctors. Some won’t except private so ask for another with your doctor that way they can’t argue with results but your antibodies could well have been having a flair up at time of test.

Gp refused to do any thyroid bloods as they were done in February they won’t do any antibodies they say their is no need but in feb ths 2.19 range 0.27-4.20 with gp bloods

Can you add the actual results and ranges for the vitamins as well please. Just being within range is often not good enough

Vitamin D needs to be around 100nmol

B12 and folate towards top of range

Ferritin at least half way in range

As you have Hashimoto's you on strictly gluten free diet?

Ask GP for immediate 25mcg dose increase,in Levothyroxine. Retesting in 6-8 weeks.

assuming you are not already gluten free, request a coeliac blood test, to definitely rule it out, before trying it for minimum 3-6 months. If it helps as many many people find it does stick on it

According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps, often significantly. Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)

Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies

Just wondering you haven't had change in brand of Levothyroxine recently? Perhaps to Teva?

Many people find Levothyroxine brands are not interchangeable. Once you find a brand that suits you, best to make sure to only get that one at each prescription. Watch out for brand change when dose is increased or repeat prescriptions

Always take Levo on empty stomach and then nothing apart from water for at least an hour after. Many take early morning on waking, but it may be more convenient and possibly more effective taken at bedtime.
