So what has your GP prescribed? All these are severely deficient
You presumably have autoimmune thyroid disease also called Hashimoto's and most likely gluten intolerance or coeliac disease
Hashimoto's affects the gut and leads to low stomach acid and then low vitamin levels. Low vitamin levels affect Thyroid hormone working
Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances. Most common by far is gluten.
According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps significantly. Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)
Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies
See GP for coeliac blood test. Assuming result is negative then you can immediately go on strictly gluten free diet. If result is positive you will need to stay on gluten rich diet until endoscopy (max 6 weeks wait)
Your current supplements are inadequate
See a different GP for correct treatment
You need full testing for Pernicious Anaemia before starting B12 injections and folic acid supplements should not be started until 48 hours after first B12 injections
Ferrous fumerate needs increasing to 2 or 3 per day, each taken with vitamin c tablet, or an iron Infusion
Vitamin D is too low. Dose needs increasing. Aiming to improve to around 100nmol. Vitamin D mouth spray by Better You is good as avoids poor gut function. It's trial and error what dose each person needs. Once you Improve level, very likely you will need on going maintenance dose to keep it there. Retesting twice yearly via vitamindtest.org.uk
