Hi Sherry, could you let us have the ranges, which are the numbers in brackets after the results? Ranges change depending on where you live and without them the numbers don't make much sense.
I am assuming your test and results above were taken 6 weeks after your new dose.
250mcg of levo used to be a common dose but if you feel your heart is racing and temp high it would be too much.
Going by your blood tests TSH and FT4 (slightly over) and FT3 are good.
If you are still symptomatic I'd ask for some T3 to be added to a reduced T4. Some people only improve on a combination of T3/T4.

Your folate (folic acid) levels are below the reference range, if I am reading that correctly. Folic acid deficiency can cause its own symptoms and is needed by the body to make healthy red blood cells. Your body does not store much folic acid so you need to eat a fresh supply everyday, or perhaps supplement until levels return to normal.
Your freeT4 is over range, and freeT3 about halfway up the range, so you have quite a whopping conversion problem. Really you need to cut that Levothyroxibe slightly and add in some T3, but that is being cracked down on at the moment so you may struggle to get it
The vitamin situation looks pretty bad. B12 and Vit A are both still low, and so is folate. This is good news in a way, because it means you will get some improvement if you can get those up, and it may improve your conversion, too.
How have you been supplementing those two? Have you been using advice from the forum, or going for possibly lower quality treatment? Do you have blood tests from before supplementing, and do you see any improvement?
Generally, if you've supplemented and it hasn't helped, the next step is to ramp up the supplements. Go for the more easily absorbed formulation, or an increased dose. For B12 there are many formats. Hopefully you've had methylcobalamin dissolved under the tongue, and a good B complex with it. For vit D you need a higher dose than GPs will recommend. Probably starting with a loading dose of 5000 or 6000 international units for a month or two.
Hopefully you will get someone more expert to comment on the details of this, and the folate.
