Please can you guys look at this for my mom? Thank you!

Hi everyone!

I finally got my mom to get her thyroid tested as her nails were ridged, she was foggy headed, her hair and eyebrows were thinning and she had a very small goitre. I can see that she is vitamin d and b12 deficient but could you look at the others? She went early in the morning and all that good stuff. To me it looks like she might be on the cusp of a thyroid issue with low in range T3 and T4 would really appreciate some fresh eyes. Could this get a little better if she sorts out the b12 and d issues? Thanks 🙏

Hellybaybee,

Your mother isn't hypothyroid yet. TSH 2.29 indicates her thyroid is beginning to struggle but FT4 and FT3 are both in range. NHS won't diagnose hypothyroidism until TSH is over range or FT4 below range. Suggest your mother supplements 100mcg-200mcg selenium daily as that is good thyroid support.

Thyroid antibodies are negative for Hashimoto's.

VitD is severely deficient. Her GP should refer to local guidelines or the cks.nice.org.uk/vitamin-d-d... Do NOT accept a prescription for 800iu which is a maintenance dose prescribed after vitD is replete >75. My GP prescribed 40,000iu daily x 14 followed by 2,000iu daily x 8 weeks which raised vitD from <10 to 107.

Alternatively, she can buy vit D3 online and self medicate. I suggest 10,000iu daily x 4 weeks then reduce to 5,000iu daily for 2 months and retest.

B12 is severely deficient. GP should initiate B12 injections and investigate whether she has pernicious anaemia causing deficiency. Symptoms are listed in b12deficiency.info/signs-an... healthunlocked.com/pasoc are the experts on PA, B12 and folate deficiencies if you need more advice.

Folate is low in range so mention that to PAS too.

Yes her Vitamin D and B12 need urgent attention. Her doctor should prescribe Loading dose of Vitamin D to be taken in split doses over the next 6 or so weeks, depending on which treatment regime is decided on. Her Folate is also low in range. I would be asking to be tested for Pernicious Anemia. Her doctor may start B12 injections straight away. She might be prescribed Folic Acid 5mg tablets to start taking after the B12 injections have commenced.

It is vital that B12 and Vitamin D deficiencies are treated immediately.

Thyroid Antibodies have tested negative for Hashimoto's. So she does not appear to have Autoimmune Thyroiditis, going by these results.

She could ask for an Ultrasound scan of her Thyroid if she has a goitre - but I would urge her to see her doctor immediately and get Vitamins D and B12 treatment started.

Don't let your mum supplement vitamin B12 until she's had all the necessary testing for Pernicious Anaemia.

Oh, and she shouldn't supplement folate until she's had her first B12 injection or her first few days of oral B12.

Thanks all, really appreciate the info, I have read your posts to her.... she is exhausted and has almost all the stomach problems associated with b12, it’s a wonder they never actually checked her. I said about a year ago that she should checked b12 and vitamin d just on the basis of absorption with her gerd and stomach problems (Crohns is also in the family) etc and the doctor poo pooed it. I could see the signs as I have the same things. I said I would go with her.

May I add that with digestive issues, Crohn's in the family and a low b12 you may want to push for some tests for your mum to exclude Celiac's. Once this is done, even if the results came negative, a gluten-free diet will help.

I actually suggested gluten free to her because we have similar things and I found it helped me (hers is far worse though)... I think she has cut down a bit and it’s helped. I’ll tell her to add that question to the list, thanks

Would definitely agree with researcherUK , insist that GP runs coeliac blood test before getting your Mum on strictly gluten free diet

Yes, go with her. Her GP should arrange the test for Pernicious Anemia and start B12 injections. He /she should also prescribe Loading dose of 300,000 IU Vitamin D to be taken in split doses over the nexf 6 weeks (or 280,000 IU over slightly different time scale). Or might refer her to a Specialist immediately.

Hi,

I am not medically trained.

Her B12 is below the range. Has she been tested for PA (Pernicious Anaemia) and Coeliac disease?

I normally post on PAS (Pernicious Anaemia Society) forum on HU. Lots more b12 info on there.
