Your vitamins are low because you are under medicated. You need 25mcg dose increase.

Your gut function is obviously poor and you may be struggling to absorb your Levo

Do you always have same brand of Levo?

Presumably you always take on empty stomach and then nothing apart from water for at least an hour after

Many take on waking, but it may be more convenient and possibly more effective taken at bedtime

You might try taking Levo with vitamin C to improve absorption

Your vitamin levels are dire

800iu of vitamin D will almost certainly not be enough

Ideally GP should test for Pernicious Anaemia and offer B12 injections (before starting folic acid)

In reality you are probably going to have to self supplement a good vitamin B complex contains folate and sublingual B12 lozenges

Only start one supplement at a time or change one thing at a time

How much ferrous fumerate are you prescribed. Presumably 3 x daily. Really you should have full iron panel and iron infusion as level is so low

Eating liver once a week will help improve ferritin too

Typical post with Low vitamins due to under medication

Low vitamins causing low TSH high FT4

If after you get vitamin levels optimal and Levo dose increased so that TSH is around one and FT4 towards top of range, if FT3 still remains low then you may, like many with Hashimoto's, benefit from addition of small dose of T3

You might also consider DIO2 gene test

Available via Thyroid UK or Blue Horizon

B12 needs to be at least 500 and nearer 9000. And 800iu is a very low maintenance dose. You could read the NICE guidelines re a booster dose to improve it or just buy your own as they are relatively cheap.
