Help to decipher test results please

Hi, please can you help

I got my latest blood test results from the Drs receptionist:

Anti-thyroid peroxidase 147 (0-75)

Ferritin 25 (10-300)

Tsh 0.77 (0.1-5) (only one I had done the last 2 times, and was 12 in December)

Free t4 18 (8-19)

Serum folate 9.3 (3-12)

Serum b12 562 (200-900)

There were others: cholesterol, est gfr, u&e, bone profile, urea, and fbc & diff.

Note I had my bloods taken at about 6.30pm as I didn't know it made a difference.

I am currently on 75mg levo, but still feel exhausted, joints all ache, no libido, cold all the time, brain fog seems worse. Hair falling out has started to reduce at least. I do have a 7 month old and breastfeeding and have little sleep too. Struggle to get sleep, or restless even if baby sleeping, and wake up with night sweats.

I did ask for ft3 and t3 and also vit D, but looks like the lab didn't process them.

Any advice appreciated please, and if I need to be taking anything else :)

Tilomilo,

TSH would have higher if you had tested early in the morning. TSH 0.77 and FT4 18 which is almost top of range indicates you are optimally dosed on 75mcg. Symptoms can lag behind good biochemistry by several months. FT3 is rarely tested in primary care and unless TSH is <0.03 labs often decline to analyse it as they are looking for signs of hyperthyroidism not low FT3 in hypothyroid patients. Thyroid peroxidase antibodies are positive for autoimmune thyroid disease (Hashimoto's). There is no cure for Hashimoto's which causes 90% of hypothyroidism. Treatment is for the low thyroid levels it causes. Many people have found that 100% gluten-free diet is helpful in reducing Hashi flares, symptoms and eventually antibodies.

Ferritin is optimal halfway through range so around 150. I would supplement iron and take each tablet with 1,000mcg vitamin C to aid absorption and minimise constipation.

B12 and folate are fine.
