I agree with galathea that excessive facial and body hair suggests PCOS.
How are your daughter's ferritin, vitamin D, B12 and folate. Deficiencies are common in hypothyroid patients and cause fatigue as well as inhibiting uptake of Levothyroxine.

Insulin resistance is generally believed to be the cause of PCOS- if this is the issue. A good diet, exercise and supplements are essential too. (not easy when you are shattered)
I am assuming the weight is a gain not a loss here and I don't know what levels of T3 are being taken. The dose of T4 is a healthy one but not uncommon so don't be concerned about this.
It is very hard to understand what is EXACTLY happening because sometimes we are alone and all I could do was have several tests done to get to the bottom of things. Thankfully my consultant(general) can think outside the box and whilst I had to have several tests privately with sheer will power I got the answers and meds that I needed.
An adrenal gland dysfunction can cause facial hair and weight issues too- ATP test.
Gut dysfunction shows if meds/foods are being absorbed correctly(stool analysis)
