Resistance-Associated Mutations in CLL Patients Treated With Novel Agents
Acquired mutations in the BCL2 gene were detected in patients who had failed on
venetoclax
. For 20% of patients, the ibrutinib resistance-associated mutations remain unknown. This data further supports the presence of alternative mechanisms of drug resistance other than BTK/PLCG2 mutations.

Update on enlarged armpit nodes
I was given a choice of Ibritunib or
Venetoclax
with eventually Ritiximab. I chose the latter - hope I've made the right decision. Starting next week with 2 days in hospital and possibly another stay. PET scan first. Ok about the treatment but concerned it may be Richter's. Scan will tell I guess.

Imbruvica-related issues: Qs + comments
* Is
Venetoclax
also considered a second generation drug? How does it compare to Imbruvica and Calquence in terms of results and side effects? * What about Idelalisib, which I’ve read is well tolerated? * If I were to discontinue Imbruvica, which other drug would you recommend?

Change of treatment
This prompted change in treatment to Obinituzimab and later transition to
Venetoclax
. Really concerned about possible side effects.

Ibrutinib V Venetoclax + Rituximab
I am being offered either Ibrutinib or
Venetoclax
+Rituximab. My consultant favours the the latter (V+R) and I will probably go with his recommendation. Do any of you out there have any views or advice to give...I would be pleased to hear from you. Many thanks.

CLL/SLL Roundtable: Updates on Patient Management (including low dose radiation treatment for early stage SLL)
The BCL-2 inhibitor
venetoclax
is also approved for frontline CLL in combination with obinutuzumab, based on the phase III CLL‑14 trial. In that study,
venetoclax
plus obinutuzumab led to significant improvement in PFS and higher MRD negativity vs chlorambucil plus obinutuzumab.

Venetoclax and Rituximab to achieve MRD?
My brother is on
venetoclax
monotherapy and he is found to start 6 cicles of Rituximab also. He is on the 400 mg dose of
venetoclax
now and his blood works are good. No nodes. Its possible to have MRD in the cases of 17p tp53 cll patients with this treatment?

CLL and hepatitis B?
I had some high hopes for acalabrutinib with
venetoclax
one day when I need therapy; now I'm not sure anymore (both these drugs are not available in Japan where I currently live; my hematologist just does not know the answer).

Worried!!
Has anyone been on
Venetoclax
and Rituximab? I am starting treatment in a few weeks and worried about side effects. I had to come off Ibrutinib after just 7 weeks. Not feeling too bad at the moment but could do with a bit more energy 😊

Question - please, and thanks to all for your support this week! Is BMB always necessary before starting treatment?
Discussed if need to treatment would be
Venetoclax
and Gazyva - hurray! However, since my one issue with this doctor is his desire to have often BMB's, can't the blood tests indicate whether treatment is needed? He will want one after we get resolution of the AIHA - hopeful here for recovery.

Any tips on moving house safely?
I am new here but have been taking Ibrutinib and
Venetoclax
for 18 months on the UK Flair clinical trial. I have been shielding since 18th March. For personal reasons I have decided to move house and wondered if anyone had any experience/suggestions on how to minimise the risks posed by Covid 19?

Walking By Faith Not By Sight
My doctor said my imbruvica is still working but mentioned a possibility of adding
Venetoclax
if Id like. I’m curious if those of you that added the meds did it because imbruvica stopped working, or to help with some symptoms. Also, is it normal for your lymph nodes to swell sometimes?

Go off imbruvica
My brother it’s now on 400 mg of
venetoclax
, for 2 weeks. All the nodes dessapeared and the blood levels are good. Because he relapsed on imbruvica, at the 50 mg dose, the doctors put him off imbruvica, in the 50 mg dose and in few days the nodes increased quickly and had fever.

Ibrutinib or venetoclax-obinutuzumab
Ibrutinib or
venetoclax
-obinutuzumab? This will be my first treatment option following CLL diagnosis. Thanks. Happy to provide specific numbers from my chart if that helps.
