Low-T is associated with worse Prostate Cancer Specific Mortality probability after 6 months of ADT: Part II
These plots show that the probability of dying from prostate cancer (after
PSA
failure) is significantly worse for men with Low-T. Since this data is after
PSA
failure, we can assume that the men in this cohort are
castrate-resistant.

PSA vs Testosterone Nadir for CRPC and Non-CRPC
This plot from Safriadi (2022) compares the mean values of
PSA
nadir versus Testosterone Nadir comparing Non-Castrate Resistant PC (Non-CRPC) to Castrate-Resistant PC (CRPC) men. For Non-CRPC men, the range of T-nadir was 2.5-49 ng/dL (mean = 21) and range for
PSA
nadir was 0.02-9 ng/mL (mean=2).

is this what CR looks like?
Here’s the
PSA
readings from that point: Nov 20 : 3.10 Jan 8: 0.31 Feb 27: 0.13 Apr 15: 0.08 So up to this point, the
PSA
is dropping nicely, as it always has with on-cycles; and as I would expect, since I’ve been a strong responder to ADT for a long time by now (since late 2015).

PSA continues rise...PSMA remains good
PSA
undetectable for two years...then an ill-advised and reckless
PSA
holiday for three years until Aug 2023 when
PSA
measured in the range of 7. First PSMA that same month entirely negative.

CAR T-Cell Therapy Demonstrates Promising Results for mCRPC
One patient in DL1 experienced a transient
PSA
response linked to early neuroendocrine expression in the on-study biopsy and 1 within the DL2 cohort achieved a
PSA
decline greater than 90% in the first 28 days post-CAR T cell infusion.

The paradigm has shifted - castrate resistance.
My T level recovered much more quickly during the last vacation than during previous vacations, and my
PSA
rose correspondingly. At 4 months my T level was 325 and my
PSA
was 4.

What is my best treatment option?
11/2023 1.3
12/2023 5.0
1/2024 20.0 I currently feel great with no pain, fatigue etc. I know I now need to resume treatment. But what? 1. Resume ADT. I was still hormone sensitive when I took a treatment vacation. 2.

Dutasteride increases PSA Doubling Time after PSA Recurrence
These two Kaplan-Meier plots compare Dutasteride (0.5 mg/day) to Placebo in a randomized, double-blind trial from 2012 of 187 men who have progressed to
Failure. In the upper plot, Dutasteride increases the
Doubling Time
from, for example, 9 months to 18 months (factor of 2X).

UPDATE
As you can see on my
PSA
curve my
psa
is now rising at a rate of approx 6months doubling time - and if it keeps this up my
PSA
will reach 2.0 sometime in the spring of next year. Then I expect to go onto some new chemo treatment.

Is it PSA recurrence or not...
Since then his monthly
PSA
results were stable, at 0.006 3 months ago his
PSA
went from 0.006 to 0.012, then the next month to 0.029. His MO wanted to see the third month's results in order to consider salvage radiation therapy. Luckily, third month's result came in at 0.012.
