If PSA decreases in MHsPC but ALP increases , is one still castrate sensitive ?
I know that PSA decreasing or remaining steady while on hormone therapy means one is hormone sensitive but if PSA decreases but ALP increases , does that still mean one is hormone sensitive ?

Does ADT work only on bone mets or organ mets also ?
Just trying to learn. Often I read that when someone is discovered with an organ Met like liver etc , they start on chemotherapy too. Do ADT and ARPI also work on organ mets or only bone mets ? Good day to all. Wednesday morning here in India !

Traveling During Treatment
My husband who had Brachytherapy 9/11 and started Zytega this month along with Orgovyx which he started in July is scheduled for IGMRT in October. We are thinking of taking a quick weeklong trip to U.K to see family for a week before the IGMRT. Going on five months in October since he started his treatment

Hypothyroid diagnosis
Hi All, I am hoping someone can help me. I used to be really skinny as a teenager and lived in a sunny country. One day my neck got very swollen and after this repeating a few times I decided to visit a doctor who said I had thyroid issues. I can't remember the diagnosis back then but he said I would

Hi everyone, I haven’t been around in a long while because I’ve been unwell and dealing with family matters. I always feel guilty popping back in after a long while but I’ve not been able to maintain any sort of consistency the last several months. Up until I started HRT I was alright with what I

When CRISPR is not enough, virtual CRISPR comes to aid! CREME, an AI-powered virtual lab
Creme de la creme...ok, bad one but I could not resist! Basically since CRISPR (even if it's a FANTASTIC tool) takes time and money, why not simulate it so we can target what has more chances to produce the desired results??? As side note, if my adt influenced memory serves me well....someone created

Nonmetastatic castrate resistant prostate cancer
I have been on hormone therapy for 10 years with stage 3 non-metastatic prostate cancer. Now I am very close to doubling the PSA < 10 months. Question. What kind of therapy should I expect when I reach a non-metastatic castrate-resistant form?

Immunotherapy before/after or concurrent with Chemotherapy
Hii all , My father is failing ADT and next step is to start chemotherapy . I have discussed the case with APAC Biotech Gurugram regarding Immunotherapy . They have suggested to do it before chemotherapy as the tumor cells will shrink during chemo treatment and they will not be able to get good quality

Need Help NOW
Hello fellow fighters! My husband has recently finished 6 rounds of Pluvicto and his PSMA PET scan has shown considerable spread throughout his bones. His PSA continued to rise during the Pluvicto treatment and the RO convinced us that it was probably just debris field from the dying bone lesions. It

ESMO 2024: Cabozantinib plus Atezolizumab versus 2nd novel hormonal therapy inmetastatic CRPC, overall survival results of CONTACT-02 Study
Basically, even if there was a prolonged progression-free survival, OS was not statistically different in the two arms. BUT a strong survival advantage was seen in patients with liver metastasis! https://www.urotoday.com/conference-highlights/esmo-2024/esmo-2024-prostate-cancer/154863-esmo-2024-cabozantinib-plus-atezolizumab-versus

ESMO 2024: darolutamide plus ADT in Patients with mHSPC from ARANOTE Trial
My take here is: patients receiving darolutamide and ADT had a 46% reduction in the risk of disease progression or death compared to those on ADT alone (hazard ratio [HR] 0.54). Secondary endpoints also favored the darolutamide group, including delays in the time to castration-resistant prostate cancer

ESMO 2024: adding Metformin to ADT for patients with mHSPC from STAMPEDE trial
My take is that there is no advantage adding metformin in mHSPC when it's low volume. It looks like there is statistically significant advantage in case of high volume, but still needs to be confirmed. (T_A will correct me if I am wrong) We will need a further trial designed for high volume patients

Life after leuprolide
We all know that delaying ADT as long as possible should be a goal and benefit for early detection for men. But with the US healthcare policy in place that is not the case for many of us here on HU as we have a late diagnosis with advanced or metastatic disease that puts on the fast track to BIG pharma

Pain, bloating and fatigue
Hi everyone. I was diagnosed last year with endometriosis and I have cysts on my ovaries. I think it's stated due to perimenopause and the HRT I have been prescribed has helped with the perimenopause symptoms, however the pain, bloating and fatigue of endometriosis are worsening.I have a consultant appointment

Cerotto Estradiol e terapia ormonale sostitutiva in vacanza

PSA rising
I had spot radiation in June for olio metastatic (2 spots), PSA was .036. In Aug PSA went down to .028. Now it’s .031. Does this mean the radiation of the 2 spots did not work? How long should I wait before ADT?
