PID ‘diagnosis’

I have strongly suspected PID for some time. I had a pregnancy loss with lots of complications for months. Whilst that part is ‘over’, at least physically, I’m left with the following:

- lower back pain

- hip pain

- pelvic pain (period type cramps)

- tailbone pain, bum cheek pain

- sometimes pain in groin and thighs

- bloating

- Low grade fever

- some nausea and chills

- increase in discharge, yellowy

Swabs are negative for bacteria but two antibiotics were started regardless, for two weeks.

Nausea and chills have gone, fever is not so consistent. Discharge still there, but I have thrush from the antibiotics so now I don’t know whether that’s more thrush related!! Everything else remains - pain in back, hips etc. Bloating worse than ever.

I had a pelvic and transvaginal ultrasound today. I was told that there were ‘small amounts of free fluid in the pelvis’. The snographer said this wasn’t enough to say PID - he would expect much higher levels, but I’m wondering if my antibiotics reduced the levels. He also said I should be feeling fine now after that antibiotic regime and I’m not.

He’s sending the report to GP but I’m wondering, what should I be pushing for now?? Referral to gynae? laparoscopy? CT/MRI? Different antibiotic regime (which I dread due to thrush!).

Still stuck in a ‘is this PID or is this endo?’ argument with myself too!

Have u heard of ureaplasma ? Only doxy and azithromycin will kill it , it’s a sexual transmitted bacteria that no one really believes in and but can cause problems in some like it did for me for 6 whole long annoying months

Hmm no, I haven’t. Trouble is, I’ve been with partner since we were 16 (now 32!!) so anything sexually transmitted is out of the equation!

Okay good to know . But sometimes ureaplasma is naturally occurring I read .. but u probably don’t have it then if you’ve been with same partner .. good luck hope u find answers

Right now I’m having Lingering lower abdomen pain..

Should go for a MRI and laparoscopy to confirm ..

I don’t think yours is the same as mine - mine radiates way beyond my abdomen to back, hips, bum, legs

My most pain and annoying is on lower abdomen, i do have legs , back and hips pain but not too bad i guess

The thrush might get under control with using a good probiotic.

Free fluid in your pelvis.....what does that mean? Retrograde menstrual bleeding?

Have you had a laparoscopy before? If not, I’d get a good gynaecologist and ask for a laparoscopy. They may push the pulls for Endo that I hear are useless. Good luck.

To update you all - A&E trip last night after being referred from a doctors appointment. The pain in pelvis was bad over the weekend and discharge getting no better at all. Spent 9 hours in hospital, finally seen by gynae. Confirmed PID diagnosis after pelvic and internal exam and after reviewing my internal ultrasound where free fluid was present. Extensive swabs taken, checking for a wide range of things - strep b was one I could remember. Given a pass to immediately return for IV antibiotics if temp hits 38 again. In the meantime, waiting for 3-4 days to start second line treatment after results are back. Also now have a gynae outpatient appointment to discuss everything. Gynae did say to me that although my blood tests show no inflammation markers, this simply means the inflammation hasn’t yet reached my blood. The scan fluid clearly shows inflammation. Just in case this helps anybody!

So glad they took you seriously and hopefully found answers! I have suspected PID for awhile now and need to find a doctor to take me seriously about PID... I have done a lot of research on this, it started with BV which I think spread through my cervix to my reproductive organs causing the PID. Also lab test shows no elevation in the white blood cell count.. makes sense it’s bc it hasn’t yet reached the blood. What made the doctors take you seriously and diagnose you with PID?

Another update from me. I’m not sure this will help anyone though as I think my situation is a bit rare.

Despite being six months on from the loss, losing early at six weeks, having two scans that show no tissue, I have now passed a piece of retained pregnancy tissue, at least 1cm in size. I think this is the source of my problems!!

I rang gynae assessment who weren’t very helpful. They agreed that my symptoms were most likely caused by that. They advised me to see GP if temp spikes or blood loss happens but otherwise, take a sit and wait approach. There is no point having more swabs, I had them on Monday! I also had urine tested on Monday! The only thing that’s been a few weeks is blood tests, but seeing as I’ve recently had metro-something antibiotic, I don’t think they’d do anymore. I will see what happens now....
