I had penis pain for decades despite several surgeries for pudendal nerve entrapment. It started when I was young like you. Aside from ruling out a sexually transmitted disease if you are sexually active, look into the Graston technique, when you take a bar and dig it into areas you might have scar tissue (scar tissue can form anywhere and doesnt require a severe injury to form). I had alot of scar tissue in the lower abdomen (at and below the belly button), butt, hamstrings, and pubis (front of the pelvis). Slowly digging into these areas and breaking up restrictions has massively decreased my pain and increased my flexibility. There are videos on youtube where you can see the Graston Technique demonstrated. And I can answer questions you might have.

You apply a cream like cocoa butter to an area, like your abs, then rub the tool over the entire area but particularly the areas that feel painful or restricted when you push on them. I had pain in the top of the head of my penis with occasional numbness on the whole thing and other pains that moved around a bit- like occasional pain in the testicles, pain while sitting, butt pain. I had a lot of different scar tissue pushing on alot of nerves but most of the pain was in that general region. It has greatly, massively, increased my quality of life imo that nerve release surgery didnt even touch. I am still seeing improvement so I continue doing it on myself for certain areas, and certain areas my physical therapist who I showed a few things, do on me, like shoulders and thoracic, where I cant reach.
Yeah my pain is weird I just woke up one day about 2 and half years ago and had redness and pain in my penis it’s like hyper sensitive to touch but and hurts at the same time some redness and it mostly feels like it’s sunburnt but sometimes shocking and kinda feels like soap in the tip and sometimes I’ll get pain in my testicles too and it seems to hurt after urination internally and after ejaculation internally it’s weird I’ve been to every dr no medicines help nothing and I don’t even wanna have sex anymore because I’m always in pain and it hurts which is depressing for a 22 year old
Have you seen a male pelvic pain physical therapist? They work wonders for patients that have pain anywhere in the pelvic area, including penile pain.
Additionally, have any of your healthcare providers mentioned prostadynia? It may be worth asking your doctor about it.
