I have been URSO for 4 months and will have bloods drawn in May. IF i am a responder (fingers crossed) and my ALP has started to lower, will my AST and ALT lower as well? Somebody told me that if your ALT is high, you definitely have cirrhosis….

I don’t have an answer for you. But I wouldn’t put a lot of credence to “Somebody told me that if your ALT is high, you definitely have cirrhosis….” unless this individual is a hepatologist (liver doctor). Best to stick to the medical experts and not worry about what may not be true.

In my experience, they all moved into the normal range over time. Don’t be frustrated if that happens slowly. As I recall it took at least a year for my numbers to reach the top of the normal range and over several years they slowly dropped to where they are now.

I started on Ursodiol in August 2023.. by February.. my ALT, AST and ALP are down to almost normal levels! My PBC was found “early” according to my GI, but so far the URSO has worked really well. I’ve also lost 21 lbs and my dr said I likely don’t even have a fatty liver anymore. I hope you have good response to the URSO as well 👍🏼

my ast and ALT were both in the 60’s.. and my all phos was at 215. By March my AST and ALT were back down in “normal” range and alk phos was down to 150’s

Hi Gaspereaux. If you respond to Urso, your ALP should lower, and your ALT and AST may as well. That depends on whether you may also be dx’d with another condition like AIH, as I initially was following a biopsy, but has since been recanted by my hep. My AST and ALT also came down into the normal range lafter Urso, so my hep now thinks I have PBC but not AIH. I’m no medical doctor and can only offer my personal experience, so it’s best to discuss with a hepatologist to be sure.

I agree. Stick with the experts and credible sources for information. If you are a responder to Urso , then the bile ducts are functioning allowing the flow of bile to the liver. Always look at the overall trend of your results - not each number. We all have fluctuations up and down. Let us know.

I agree with general remarks. I think that there are many different cases and overlaps and best not to get ahead of oneself. In my case my ALP tends to be high normal range but others high high and I have some cirrhosis which makes me wonder why some consultants just look at ALP? The unquantifiable parameter is "Do you feel well or ill?" Very best of luck.
