All dependant on what the situation is and where the patient is living. Assessments need to be made and each Department will work in conjunction together. I remember when an Aunt of mine had Dementia, Social Services was contacted by Her Doctor. They also contacted the Mental Health Team for a assessment, before referring to other departments. They all met in a Nursing home where she had been taken earlier on recommendation by Social Services. I was representing my Aunt at the Nursing home, Mental Health were there, the Nurse and Manager of the Nursing home, The CPN was in attendence,, further representations where made by the Mental Health Team to other relevent departments and at that time discussions, regarding payment s and if the patient had sufficient money to pay etc. At various times other services like the Court of Protection needed to be approached to look after the estate of the person etc. In initial stages if the patient is still at home the Social Services would supply assisistance etc, Mental Health would also help to watch over the persons mental health. In the past meals were provided or a Day Centre would be contacted and the patient would be taken for looking after on a daily basis ??.
If the person was in hospital various departments would work together and help with assesments. Many different problems will need to be attended to.
They would again work with Social Services and Departments would again need to discuss together the best way forward.
Normal Mental Health Patients would initially be assested by the GP and Mental Health. The patient may be Sectioned, various levels and would need to attend or removed to various levels of treatment. on Wards or Day Centres Not all patients will be taken onto the Wards. i depends on if the Patient is any danger to themselves or others.
All situations are not the same, it is difficult to explain the various Pathways. You are on a Course it will be all down to what the situation is at that time
