The doctor said my situation is very unusual and he cannot make a conclusion

Hello everyone!I am 27 years old, unmarried, and have an imbalance in my hormone levels. I had male breast development (gynecomastia) at the age of 13. By the age of 20, there was no further change, so I underwent surgery to remove the breast tissue. I have prominent male characteristics, including an obvious Adam’s apple, facial hair that requires daily shaving, nocturnal emissions, and morning erections.

Sex Hormone Panel:

Progesterone (PROG): 0.231 ng/ml

Testosterone (TESTO): 1.46 ng/ml

Estradiol (E2): 29.26 pg/ml

Luteinizing Hormone (LH): 22.88 mIU/ml

Follicle-Stimulating Hormone (FSH): 34.35 mIU/ml

Prolactin (PRL): 16.21 ng/ml

25-Hydroxyvitamin D (25-OH-VD): 13.97 ng/ml

Free Testosterone (FT): 6.58 pg/ml↓

Testosterone (TEST): 1.600 ng/ml↓

Follicle-Stimulating Hormone (FSH): 34.80 mIU/ml↑

Luteinizing Hormone (LH): 22.20 mIU/ml↑

Inhibin B (INB): 12.28 pg/ml↓

Chromosome Test Report: 46, XY, with no deletions in 15 AZF gene sequence-tagged sites.

Testicular Ultrasound:

Left Testicle: 1.5 x 0.9 cm

Right Testicle: 1.5 x 1.0 cm

Left Epididymis Head: 0.7 x 0.6 cm

Right Epididymis Head: 0.6 x 0.6 cm

HCG Stimulation Test:

Pre-stimulation: Testosterone (TESTO) 1.46 ng/ml

24 hours: Testosterone (TESTO) 4.53 ng/ml

48 hours: Testosterone (TESTO) 4.23 ng/ml

72 hours: Testosterone (TESTO) 4.13 ng/ml

Semen Analysis:

Liquefaction Time (LIQUE-TIME): 30 minutes↑

Total Sperm Count (QUANTITY): 34.80 million↓

Sperm Concentration (DENSITY): 8.70 million/ml↓

Sperm Vitality Index (INDEX-VITA): 0

We are not doctors in this forum. You really need an endocrinologist to interpret the results for you and ensure that you do not have a problem with your pituitary gland.

Your low testosterone and high LH and FSH could indicate hypogonadism. Your smaller testicles, low sperm concentration and total count and no sperm vitality suggests impaired spermatogenesis and testicular function. Your vitamin D deficiency may contribute to general health issues and impact your overall hormonal and metabolic health.

When you said doctor, is it a specialist or a GP? You probably should get a referral and consultation with a specialist in endocrinology or reproductive medicine to help you determine the cause and see what the next steps are if you haven't seen one already, good luck!

I saw an endocrinologist and took vitamin D in September 2024.
