So I’ve had balanitis for quite a few years that comes and goes in severity. Just a rash on the gland but not had any other symptoms.
Last year it started to burn when I urinated. Had std tests and all negative.
The burning is still here and that changes in severity also.
However, I have recently noticed that when I pull my foreskin back and urinate the burning is gone.
Does anyone have any idea why this is?

If you do not pull the foreskin back to pee then you are likely to get some urine trapped between the foreskin and glans. This can cause pain in a glans that already has problems. I always retract my foreskin to pee.
Before my circ' I never pulled back to pee and that in-itself never caused a problem.But it is possible that your foreskin is causing the issue. If your foreskin retracts freely then, although I hate to suggest it, a circ' may be right for you.
But try creams like sudocrem or consult your Doc' for referral.
I, too, have had balanitis for a few years. I also have urethra issues like you. I don’t have a silver bullet, but here’s what helps me: for balanitis, I dust up with cornstarch (yes, cornstarch!) a few times per day. If it costed $100 instead of $2 I’d still buy it . Right now, for the urethra, I just drink lots of water. I never noticed urethra issues until I started drinking less water this year when I retired from my job and its close water cooler!Dermatologists and most internet advice have been very little help, so I’ve found these by trial and error.
