Should I get circumcision at 32 years old?

I was tested positive with high-risk hpv (33) several months ago and have had phimosis since I was born. I have been using mometasone furoate to treat my phimosis, but after 2 weeks of using it, I have only been able to retract my foreskin when flaccid and it still feels tight.

Now I am aware that phimosis and hpv are risk factors for penile cancer. My question is, should I get circumcision in order to reduce the risks of penile cancer at my age?

After reading these studies though, I have been growing concerned about getting circumcision.

"In contrast, there was some evidence that circumcision in adulthood was associated with an increased risk of invasive penile cancer" (ncbi.nlm.nih.gov/pmc/articl...

Sorry, I'm not native English speaker or writer. It is often difficult to understand the nuances of text. I just wonder, do you really have had HPV or your doctor in Korea has done some genetical test to show you the risk factor.

You asked earlier about HPV vaccine like 'Gardacil'. It is on our national vaccination program for teenagers here in Finland. The idea is to give them three injections before they start to having sex. If you already have had HPV, vaccine doesn't protects you.

Penis cancer is very rare 0.84 incidence per 100 000 person-years. I'm not talking about prostate cancer now.

As long as you can maintain daily hygiene, the risk of penile cancer is not valid reason for circumcision, IMO.

Thanks for the reply. To answer your question, the urologists here in Korea apparently do a swab or urine test and can detect which hpv strains a patient has. I thought this was weird because I know that in America Hpv testing is not performed.

Penile cancer is rare in the general population of men but it is a higher rate in men with Phimosis due to the inability to remove smegma and debris between the glans and the foreskin. Cancer of the penis is almost exclusively found in uncircumcised men, but circumcision as an adult does not change the risk of having penile cancer as it is only effective in those men circumcised as infants. You said that you can drawn back the foreskin when flaccid, that means you can clean your penis, and the risk of cancer is reduce to that of the general male population. I am going off my knowledge, I am not a Medical Doctor. As for HPV, just about everyone is exposed to that virus in their lifetimes.

As for circumcision, it is the problem of the foreskin's inability to retract by itself or by your hand when your penis is erect that is the serious problem. However you may be use to that during sexual penetration, and the problem of tearing of the foreskin may not be an issue for you.

I was circumcised as an adult, no problems with my foreskin, and at my choice, but I would say for you to not get circumcised as you can draw back your foreskin when flaccid. It is the sexual aspects that may warrant circumcision. If I had a son in your situation I would advise circumcision but my son would not be aged 32 and with Phimosis. I have three sons.

Have you considered trying to stretch your foreskin with the help of a prescribed steroid cream?

Keep treating the tight ring in conjunction with stretching It might take another month, but you’ll achieve full retraction with an erection. The odds are pretty slim that you’ll get cancer. Just do regular checkups. If you get new partners, wear condoms to prevent spreading the virus.

I would say go ahead a get it over with. I’m 24 and I was starting to get some type of phimosis every time I had sex it would tear and it would just get tighter and tighter. So I decided to get circumcised and so far I haven’t regret everything feels the same.

Out of curiosity, how do you think you contracted HPV? By means of a sexual partner you are attached to or was it a random thing?

Yes, take pictures of your penis before, take picture in flaccid and erect state, and take measures. Let your urologist know your erect state measure. Tell him that you don’t want it too tight the penis shaft, tell him you want it a little loose penis shaft skin. Try to use a permanent marker and mark where the glans begin, do this in erect state. Try to make things easy for the surgeon. If you forget this u could end up with too much skin removed, and maybe lose 5cms length in erection. Google circumcision types and you will understand what am talking about. Good luck
