If fragmin = heparin which I think it does it has a 12 hour half life. That means it will reach its peak in 12 hours and reduce by half every 12 hours after that.
Warfarin has a 72 hour half life, ie it takes 72 hours to reach its peak then reduces by half every 72 hours after that.
Agree - 12 hours is the norm.
