I have a day procedure coming up next week and I'm meant to bridge with clexane. Stop warfarin 3 days before surgery and start clexane. The doctor didn't say what to do at the other side of the procedure. Do I take both clexane and warfarin until my INR is back in range?

You really need to check with your Doctor, I had to have both until back in range but everyone is different
I agree with this and if you have it to hand, they can't fail but to abide by it and or call the duty Haematologist for further clarification, as even with the best setting up, it can get lost in translation between staff and shifts with dire consequences. MaryF
I doubt very much the hospital will have any idea as they don't seem to have any experience with APLS.
It's a day procedure so it will be up to me really what I do unless something goes wrong and I have to be admitted for longer.

I was that person tassie and I showed the ward the protocol but they just insisted the medication was not prescribed even thou physician had given the ok . Finally got back on tinzaparin (clexane ) and warfarin 33 hours later which was worrying. Hope yours goes ok
