Hi, I have only just sent my form and evidence off to apply for PIP, so am sure others who were successful can help.As for me, I got copies of letters from my rheumatologist ( as I have inflammatory arthritis too ) but he wrote also has fibromyalgia.
I also had two letters from a pain clinic who were the ones who diagnosed me with fibro.
I also accessed my own GP records ( ask your drs surgery ) and got a friend to print off the notes I felt were relevant and sent those too.
a tip someone have was to take a photo of all the completed forms and letters for your own records. And I sent it recorded post too.
Am sure others can are more advice
I had to provide Dr's letters, consultant letters and go for a check up with 1 of their Dr's. It seemed to take for ages but in the end I got it
HiThe form is quite long and you need to know how to answer the questions. If you can get help and support from someone such as CAB, Age UK, Welfare rights,. There are websites that help too such as this one. They have a benefit adviser on here. Benefits and work is another one and one that I use constantly.
Also as well as professional letters ask someone who knows you well write one saying how it has affected you and the changes you have had to make.
It isn't just about your diagnosis but how it affects you and the changes you have needed to make in order to carry on.
