Hi there, has far has im aware you have to request them from your GP. I have always sent a letter to our practise manager with what I want along with the date/dates. I have never had any problems getting them. They were posted to my home. Other surgeries may have different practices. I would ring you surgery and ask them.

If it is records from a hospital you require, what I did here (I'm in N. Ireland) was to ring the relevant hospital and ask to get put through to the Records Department. I then told them what I required. They sent me out a form, which I filled in with the names of the consultants I had seen, approximate dates etc, sent it back (there was a fee to pay, I think it was £20) and they posted the required information out to me. x

If you need them from the hospital, there is a health records department and the secretary of the neurologist should be able to tell you where to find them or give you a number. You have to fill in a request form and it helps to state your NHS number not hospital number, pay a fee usually £10 for latest admission and they will advise you when to pick up. Otherwise, the discharge summary usually is printable by the secretary or doctor. Hope this helps

I have recently done this

I got in touch with the patient records department at the hospital I filled out a form requesting records of different episodes of hospital MRI s CTS xrays and reports they have 30 days to produce information I had to give dates photo I'd and current proof of address I then got the documents and DVD disc of images .I was not charged a fee

With my gp I also asked for my records again they had 30 days to supply me with them again there was no charge for this .

I would tell anyone to do this I was amazed at the information I've been able to gather good luck hope this helps many people Diddy1965 👍

I would go to the hospital to medical records and ask for them. Here in the US we have a right to obtain our medical records. I’m not sure about the UK but I would think that you would have a legal right to your own medical records.
