can anyone tell me if GP will supply me with treatments and diagnosis proof so that i can send with PIP claim and if so did you have to pay
information from GP: can anyone tell me... - Fibromyalgia Acti...
information from GP
Hi goldilocks54
Many GP's will do this but they may charge you for a private letter. If you discuss the issue with your GP prior to making a claim you may get them to agree to allowing the DWP to write directly to them for a medical report at no cost to yourself.
If you are currently not receiving treatment for your Fibro I would discuss this with your GP.
I want to genuinely wish you all the best of luck with your claim for PIP.
All my hopes and dreams for you
Ken
Hi
Would suggest that you talk to your Gp
As The Author says there may be a charge
My Gp charges about £20 for an insurance form to be completed
Good luck
I didn't supply any evidence. Just ticked the box that they could contact gp and gave them gp address and phone numbet PIP got in touch with the gp themselves..
Hope this helps
Hello Spanner, Heartening if that approach worked. II've just done the same as you (because otherwise my surgery charge £110 for a private report).
Did you succeed, I wonder (guessing the answer is yes?)
Thanks for clarifying this.
Yes it worked. Had to go for a medical with the PIP people, not as scarey as I was imagining. The lady was really helpful too. They go through what you can /cannot do round our home, what help you need to prepare food, shopping, how farmhouse can walk ect.. If your successful it can take a further 4wks. Got mine within a week. Got it all back dated to when I initially put my PIP claim in,. It took well over 7months. Hope it all works out. Its a long waiting game, so to speak
Gentle hugs
Hi I dont know who did your diagnosis, if it was a Ruematologist or some one else other than your GP you can ask him for a copy of the report sent to him.

Most GP's will give you a short letter outlining your health conditions and treatments. Mind just said what conditions I had and that they had been over a lengthy period. He just added that I had difficulty sitting on a hard chair for any length of time as he had noticed this at consults. He also added to the back a list of my medications. Some might make a charge like they do for Insurance Claims, etc.
I know when I helped people to complete Attendance Allowance forms some doctors would not complete the form sent to them as they said they were not being paid enough to do it and were too busy. No comment on that one by me!
You can get access to your medical records but normally you have to request that in writing and state what period you need them for. I believe if the information has been put there in the last 40 days this should be free but before then there is normally an admin. charge of about £10. It can take up to 40 days to get these records but the guidelines say they should be available in 21.
I know I asked for the records of my consults with the physio at the hospital and he said that unfortunately he wasn't allowed to give me a paper based copy but if I wrote to the hospital they would provide it.
As Spanners 3 says if you trust your doctor to do it just tick the box to say they can contact your GP. Just a warning from my own experience. If you are turned down for PIPask your doctor for a copy of the letter he did. I was initially turned down for DLA based on my form and GP's report. When I phoned upset and asked what he had put he asked to ring me back. When he did he was all sheepish and said he had never actually filled it in as it had got caught up with some other paperwork in his pigeon hole and he would send it at once. I wrote to them saying how could they base their decision on something they had not received and how much the tribunal judges would love that incompetence and within a week had a letter saying they had reviewed my case and decided to give me DLA.
Good luck let us know how you get on.x
It's £20 at my doctors but pip normally get in touch with your doctors and when you go for medical make sure you take someone with you or if you have a support worker they are normally better to take with you...
I asked the receptionist for a copy of my repeat medications and also the list of conditions and when they were diagnosed. She did this whilst I waited and didn't charge me anything as she said it was my information and I was entitled to a copy of it on request.
thank you everyone for all replies and advice . x
