OHSS after embryo transfer

Hello,

Hoping someone has been in the same boat as me and can offer some advice or reassurance.

I had my egg collection on Monday and my estrogen levels were quite high. I was prescribed cabergoline prior to this and advised to take until after transfer.

At transfer yesterday my doctor said my follicles were still pretty large and filled with fluid. However, my bloating and other discomfort have decreased and I've felt pretty good today.

However I've read that if you fall pregnant, it can worsen OHSS. So does this mean my transfer has failed since my OHSS symptoms seem to be improving? To reiterate my transfer was yesterday so very early days and I took my last cabergoline last night.

Hi there, I don't have a personal experience but my sister did and it ended well for her. She did the transfer while she had no ohss symptomps whatsoever.

She had a BFP and when she was 6 weeks pregnant she had severe abdomen pain and had to be rushed to a&e a couple of times. All they did was to manage the pain and she was told that baby wasn't in danger.

She has now a 4 year old girl from that pregnancy 🙏.

Hi there,

Hcg is what is responsible for worsening ohss symptoms in pregnancy. Being only one day past transfer your body would not be producing hcg yet so it doesn’t mean your transfer hasn’t worked.

But you may find that if you are pregnant in 5 or 6 days and when your body starts producing hcg that your ohss might worsen a bit.

Good luck! I do have experience of this, at transfer I had some fluid, but felt fine, so they weren’t too concerned so went ahead.

My transfer was successful and I’m currently 27 weeks. I developed OHSS symptoms at around 5/6 weeks when the HCG was higher. I initially thought I had appendicitis so went to A&E. I was admitted for 3 nights, to monitor fluids and pain relief. My ovaries were measuring around 15cm. So very swollen. Baby wasn’t effected at all.

Unfortunately, there isn’t really a way to avoid OHSS, but keep active and drink electrolytes, plenty of fluids and if you feel any symptoms then seek medical help straight away.

Hi, I recently had a 3 day fresh transfer but ended up having to go into hospital with OHSS two days after the transfer, so when it would have only been forming into a blastocyst. At the time I asked the doctor if it would affect my chances and they said not, but I felt a lot better a couple of days later and unfortunately it was unsuccessful.

My understanding is that it is only late onset OHSS, that you develop when HCG levels start to rise, that is a predictor in pregnancy. I think early onset OHSS, which I had and sounds like you too, is not a predictor in whether you are pregnant or not.

Long story short, my advice would be try not to read too much into it. I was told that I might recover from the early onset OHSS, but not to be disheartened if that was the case, as it can completely go away altogether and I could still be pregnant, or it might come back as an indicator of potential pregnancy.

hi, I also had an egg collection last Monday and transfer yesterday, also had the tablets too.

No it doesn’t mean your transfer has failed, it just means your ovaries are still recovering after collection (fluid build up is expected). Your ovaries will be swollen for a number of weeks too.

the reason why OHSS can worsen after transfer is that if the embryo implants, hcg starts to produce. HCG can worsen OHSS so sometimes people notice symptoms about 5 days or so after transfer or end up quite sick thanks to the OHSS.

it doesn’t mean your transfer will fail if you develop OHSS but obviously being quite poorly is best avoided if possible in early pregnancy.

Do you have any symptoms? Keep hydrated, eating salty food and using electrolytes to help with the fluid build up. Lots of rest if you can. If symptoms ramp up you’ll need to speak to the clinic.

The Only way to know if your transfer is successful or not is a pregnancy test in 10-14 days time.

I developed late onset OHSS, no indications that i was at risk after the egg collection or on transfer day. It took 3-4 days after transfer before i realised something was wrong. The first few days i had no symptoms whatsoever. I eventually took a test at 6pdt and it was positive. My symptoms got progressively worse and my OHSS was confirmed at 9dpt. I'm currently 32 weeks pregnant from that cycle.

Hi, Like you my follicles were still inflamed and had some fluid present at transfer. But it wasn't too severe, so they went ahead with the transfer. I did have some bloating and discomfort, but that was before my egg collection and by the time of my transfer, it had subsided. However, I ended up with late onset OHSS and spent a week in the hospital. OHSS won't affect your baby at all, but you'll feel like crap. My baby is almost 5 months old now though and was totally worth all the drama lol.

The symptoms didn't present until 8/9 days post transfer. If you only had your transfer yesterday, its far too early for you to be getting any symptoms as late OHSS occurs due to your body producing HCG to sustain a pregnancy. If you don't get it, it doesn't mean you're not pregnant. Your retained fluid may have just already resolved itself so try not to rely on it as an indicator.

In the meantime, continue to eat healthily, drink plenty of fluids, and take any prescribed medication. Good luck with everything. Hope it all works out for you x

I’m in a similar position, estrogen levels a bit high so started in cambergoline yesterday and EC is tomorrow. Been told to take cambergoline for 7 days and I’m hoping levels go down. Choclover21 how you feeling now?

I feel pretty normal now. I took my last cambergoline on Sunday I think and haven't noticed any symptoms except from the progesterone pessaries.

Worrying about lack of symptoms now! 😫

Ohh no, try to stop the worrying (I know easier said than done!) some do/don’t have any symptoms on my positive I didn’t get any. All the best. 🙏🏽

Yes absolutely! Trying to have a positive mindset but also know I have frozen embryos so I have other options if this doesn't work. Still so tricky!
