Just wondering anyone that has had endometriomas and IVF how big they were to continue with IVF?
I have had 2 opinions from two hospitals so feel confused what to do! 1 has advised to have the cysts removed and then go straight into IVF whilst the other is reluctant to do surgery due to my AMH but I don’t qualify for NHS funding for another 5 months. I am waiting to see this one again after having my AMH and AFC scan.
I have 2 cysts on my right ovary 6.5cm and one 3.5cm. My left ovary is accessible but my right isn’t because of the cysts. I had a bilateral cystectomy last year and my AMH in 9.7.
Any advise would be appreciated!

Hi Amy2106I had a large 9cm cyst removed from my left ovary last October and a smaller right endometrioma drained before my third round of IVF. My surgeon also made my right ovary more mobile for IVF. I still have three right endometriomas (all 2cm) and one left endometrioma (3cm) and the fertility clinic and endo surgeon both advised not to do further surgery for these as they are below 5cm (they said that's the threshold) and that the risk of damaging my ovarian reserve is too great. So my clinic is happy to go ahead and do IVF with the remaining endometriomas there and they will work around them for the egg collection! They managed to access all my follicles during the last collection despite these endometriomas so I'm not too concerned. Hope this helps!
hey! Don’t have the answers but I too have an endometrioma which they seem to think is too small to worry them. Just wanted to share this insta live that is super helpful about fertility and endometriosis - instagram.com/reel/C5X-MY1P... might help answer your question, best of luck x
hi Amy! Sorry your going through this, I had an endometrioma appear just before my first FET after my egg collection and first failed fresh transfer. I had such conflicting options too. In the end I went ahead with the FET as they said my ovary had done its job but if I was to go on to have another collection they would recommend it being removed.
My FET failed so I had the laparoscopy and waited the recommended 8 week before having another collection. Sadly that round didn’t work either but I think there were other factors that contributed to the failure rather than the surgery.
I went private and had another collection and my second FET from that round is now my 14 week old little girl.
My surgeon was amazing and told me she would 100% recommend having it removed to increase my chances
Hope all goes well and hope I’ve helped a little xx
It really is! And it doesn’t help when you have conflicting opinions. I am due to speak with the fertility nurse next week so hopefully she’ll give me some helpful advice!
Hi I had a 10cm endometrioma removed in 2019 after beginning my first ivf cycle they found it and it had to be removed due to it's size. I then went back to have further ivf and I had another endometrioma around 4cm , they proceeding with the treatment as they don't like to operate multiple times incase of affecting amh too much. Ive had four egg collections in total all with endometrioma on my ovary and I am currently 29 weeks pregnant. I know they like them to be measuring 5cm or less. the cysts didn't cause any problems during treatment but they always had difficulty accessing my left ovary. they did manage to though.
It is difficult to know what to do for the best with having them removed because more often than not they reappear. If they get too large then they have to be removed but smaller ones I would say are fine. all the best X
Hi! I have kissing ovaries and a 5cm cyst on one. During private IVF (just 1 cycle!) they could only collect from a small section on one of my ovaries. They only managed to get 2 eggs and both died the next day. I was told they were discoloured.... possibly due to endo? They were not sure. I was also a slow responder to stimulation, again most likely due to the endo. I have only ever had a diagnostic lap, where all they did was drain 1 chocolate cyst. That was 7 years ago. I have never had any endo removed. I am now hoping to go the surgical route to get it removed and try again with my own eggs. I took all the supplements and dietary advice that was advised on this platform and was still unlucky. In your case it really depends if your endo has impacted the egg quality. It doesn't hurt to try ivf first and see if they can access anything. Good luck x
