talk to me about thyroid (& WBC) levels! 🤓

Hi 👋

Just had the blood tests I was sent for after 2 consecutive miscarriages but don’t have an appt to discuss for a couple of weeks. Although I’m a fully qualified Google dr 🤓 this will drive me nuts so wondering if anyone has any thoughts on these.

TSH is 2.65 and T4 is 12.7. I know they like TSH under 2.5 for fertility. It’s pretty close so is this a quick/easy fix? Who do I ask to treat this - my GP or fertility clinic?

Another thing was a raised white blood count. It’s 14.9 so not crazy high but this has come up before when I had unrelated tests about 5 months ago. I had no other symptoms of infection so they just let it go but now I’m reading that elevated wbc can be a sign of an autoimmune issue or raised NK cells. I know nothing about these so any thoughts welcome xx

Hi

I don't know how much help I can be but I have found my GP was only willing to support me lower my TSH after it went above the NHS threshold. After that point they became more amenable to bringing it down in accordance with fertility recommendations. But prior to it exceeding 4.2 (the NHS cut off) they didn't want to know and therefore any meds were prescribed through my clinic. Of course every GP is different and you may have a more supportive one. My GP also asks for something in writing from the fertility clinic as to what TSH should be.

I can't comment on the white blood count I'm afraid xx

Hi Solly-44 In my case my Fertility Clinic gave me the prescription. But mine was 4.1 first blood test and 4.6 second one, so they said that I need put it down before starting the stims.

My TSH was 3.2 when the clinic tested me prior to my first IVF, it was already over 2.5 when the NHS tested it, but they didn't really do anything about it as it's not over the threshold that the NHS considers for medication. The clinic gave me the prescription for levothyroxine themselves and they were ok with me starting the first cycle at the same time as starting medication, I didn't have to wait for my levels to go down.thr clinic then refilled it throughout the IVF. After my positive test (quite a few months later), when I moved to the NHS For antenatal care, the clinic wrote a note for my GP explaining their recommendation, and my GP was happy to issue a repeat prescription after that. I have a consultant who monitors my thyroid function. After I had my daughter I lost 3 stone in 12 weeks without trying (I was loving life) and then I went the other way and put on a lot of weight and I just felt something was off. Had lots of blood tests, some not routine and turned out that although my thyroid function came back borderline I have thyroid antibodies which can cause miscarriage. It's unknown how long I'd had them, apparently not triggered by pregnancy, but now have to take thyroid medication to keep it in control and hopefully prevent a miscarriage.

My TSH was up at 7 when I had my miscarriage, but was around 3 before getting pregnant. My GP was not helpful so I paid to see a private endocrinologist who got my levothyroxine increased and she wants TSH to stay around 1. Did you get a range with your T4? She like to see mine about 75% up the range.

I've also recently had a biopsy done to check for immunology issues- I have high thyroid antibodies and too low NK cells which I didn't even know was a thing. I'm getting steroids and other meds for my next round. There can be a link so It's worth having a full thyroid panel done if you haven't already.

Hi,I spoke to a clinic who said they prefer the TSH to be under 2 and said I had to speak to my doctor and like everyone else has said, my doctor wouldn't do anything as I am within range. I have been speaking to a holistic health company who have referred me to a private doctor to help.

If you can get your clinic to prescribe the meds that would be ideal for you.

Hi Solly, I'm really sorry to hear about your experience with miscarriage. It's draining.

Just a comment on your WBC, whenever my levels went above 10 during treatments / TWWs / early pregnancy, my consultant offered a round of intralipids and wanted to bring them back down to below this number. Like you say 14.9 isn't sky high but it is still elevated with no backup reason.

Have you looked into LAD testing / LIT treatment?

My background is I have unexplained infertility, had 2 ICSI cycles, frozen transfers, failed implantations and chemicals previously...

I had my NK cells checked and they were ok, but after having a miscarriage at 12 weeks where my WBC was raised throughout, the results from the testing after the D&C showed a perfectly healthy baby, it was thought I was having some sort of negative, immune related response to my attempted pregnancies / implantations.

On my consultants advice I did the LAD test and the follow up treatment; a course of LIT. I don't know if you've heard of this but it's what has worked for me. Immune issues are so tricky and a raised WBC is one of the markers my consultant really focuses on and one that I've tried to manage throughout. However other consultants don't rate this type of treatment at all, so it really depends. I know fertility issues are so individualised so this might not be relevant to you. But I know the feeling when you are at a bit of a loss as to where to turn next. I hope you get your answers and some clarity...
