Progesterone to prevent miscarriage

Does anyone have experience of taking Cyclogest to prevent miscarriage? I’m 12 weeks pregnant through IVF and since my ‘natural’ progesterone level is basically zero I am really worried about coming off progesterone- which my IVF clinic has told me to do at this point.

I’ve weaned down to just being on Utrogestan twice a day (was on loads more previously) but it runs out soon. My obstetrician said he was happy to prescribe me more progesterone to get to 16 weeks - but he only has experience of Cyclogest. But I’m worried about taking that vaginally. I know I could take it through the ‘back door’ but the NICE guidelines say vaginal pessaries specifically so I don’t know if that work well. Has anyone been given Cyclogest to prevent miscarriage- and if so how did you take it? Many thanks in advance x

Hi. Congratulations on your pregnancy. I am pregnant with twins and have continued with progesterone. Now just 1 x cyclogest a day since 14 weeks until 28 weeks. NHS consultant prescribed this once I ran out of supply from IVF clinic. I have always used it vaginally and had no issues ( a bit messy but ways to manage this ). Hope that helps x

Thanks for your reply - it’s good to know others have taken Cyclogest and it’s worked well. Congratulations on your twin pregnancy. Do you know if having twins is why your consultant has suggested staying on progesterone until 24 weeks? x

Sorry to jump in your post.. I am just wondering how you went about getting g the cyclogest prescription? My IVF clinic is abroad and I have asked my midwife as I do not have an obstetrician.. she is not sure how to go about arranging a prescription. I am 10 weeks and will run out of cyclogest in 2 weeks. I fit the NICE profile and have asked for progesterone supplement. Thank you

No problem- it was in discussion with an obstetrician after my 12 week scan. I am obstetrician led as I am high risk (not to do with progesterone, for other health reasons). Most people are midwife led. But I would have thought your midwife would have access to an obstetrician who she could ask to prescribe it. I think GPs might also be able to do it, but I haven’t tried this (yet). I would ask your midwife to refer the request to a doctor. Best of luck with it x

Agree with JuicyLu. I was running out of fragmin and midwife asked the Dr who was in clinic; however I saw the midwife in hospital so might be trickier if seeing midwife in community setting. I would recommend emailing the midwife x

Thank you very much. Yes I am midwife led but my midwife seemed to be a bit confused as to what to do. I will wait for her to get back to me. Good luck with your pregnancy x

I ran out of cyclogest at 9 weeks after treatment through the NHS. I asked the clinic for more they said no, so I spoke to my GP who said they can’t prescribe it without the consultants say so. Again the clinic refused but I’m 17 weeks now

I am 10wks still on progesterone suppositories twice a day. Morning I do rectally and pm I do vaginal mine are also cyclogest. I am hoping to come off mine by 12wks because I feel dreadfully out of breathe and I am positive it’s the progesterone!

My shortness of breath was much better after I came off progesterone so should hopefully be the same for you! I felt like a 90 year old on it 🥴 x

Omg it’s horrible I can’t even stand I have to lean over 🥴 but I’ve been dealing with HG it’s brutal….but I am grateful!x

How you coping with the HG? I’ve been on every medication going I’m still not brilliant but I have to go back to work as I can’t afford to live on SSP, I’m dreading it as I’ve been off for 2.5 months luckily I work from home but still. Hg is beyond brutal

I am so so sorry your going through it too 😭 my doctors have been very good and tried me on 3 different meds before finding one that helps loads! It’s a controversial med called domperidone but it’s the only one that’s allowed me some relief enough to eat and drink. When I’ve taken it and then just get what I would consider “normal sickness” I still vomit but only a couple of times and I don’t feel rotten with it. I honestly think getting the right medication is key. Fingers crossed it’s working right now for me 🤞🏼🙏🏼 Luckily in a position where I don’t have to work because I don’t know how I’d of done it….,tbh I wouldn’t of coped xx 😘

Thanks - that’s interesting about being out of breath. Ive felt very tired and get knackered walking - maybe that’s partly why?! x

To reassure you, I felt so sick, weak and unfit until about 16 weeks but the shortness of breath and sickness eased and I can now walk normal distances again so I don't think it's all the progesterone as I am still taking it. Perhaps it is just the intense mix of hormones increasing rapidly in the beginning that can make you feel worse.

Yes after I looked back at symptoms after transfer it started just after implantation! So the increase of pregnancy progesterone and added meds of progesterone I am positive that’s what’s happened. I literally can’t stand to brush my teeth or boil the kettle 🥴 and I take a break up the stairs lol like Kiko said like a 90yr old lol xx

Hi,

I was on cyclogest too up until 12wks. The clinic be told me to check at my scan if the placenta was working ok from what they could see. If all was fine I finished what I had open, one a day and then stopped. I stopped them just after 12wks and am now 14wks 4days.

I think you need to do what feels comfortable if you know your levels are low see if you can get them checked to maybe help you feel at ease or just ask to continue. As someone else mentioned they are a little messy vaginally, which is what I did, but it is manageable.

The last thing you. We’d us to feel anxious or worried as that isn’t good for you.

Thanks - and congratulations on your pregnancy. You’re right, it’s better to feel comfortable with what we’re doing. There are many things I find I’m worrying about but I’ve got a bit obsessed with progesterone as it’s naturally low for me and want even that high when tested pre-transfer, despite taking lots of progesterone at that point x

I was weaned off Utrogestan at 10 weeks and took 400mg of Cyclogest anally until 16 weeks x

Thanks for this - that’s really interesting as that’s basically what I am doing, although different amounts (800mg Utrogestan to 13ish weeks, then 800mg Cyclogest to 16 weeks). Do you mind me asking why they switched you rather than keeping you on Utrogestan? In my case I think it’s just because that’s what my obstetrician is used to x

I conceived naturally which was a complete shock shortly after my second failed ivf. At about five weeks I had a bleed and the hospital said to start taking cyclogest again. As I was born with two Cirvix (long story) I was kept on this till 28 weeks. My daughter is nearly two and happy and healthy, while having to take the presserie (was advised by my doc back door application) was a nuisance is strangely gave me piece of mind xxx

First ivf pregnancy I took my cyclogest vaginally and hated it, had to lay down for 30 min and it was messy.With my second ivf pregnancy I asked if I could take it rectally and my doctor said it's fine and so I took it rectally and loved it, no need to lay down and no mess.

Was off progesterone both times around 12 weeks

I guess they’ve told you to come off the progesterone because the placenta makes it at 12 weeks (rather than your ovaries that normally would). No harm in you taking more though and I don’t think taking it vaginally would be a problem.
