I got the results of my Liver Biopsy and it revealed mild NASH with stage 1 fibrosis. I was scared of possible cirrhosis because my fibroscan showed S3/F4. So my biopsy had better news in my opinion. All I really need to do now is eat healthier and exercise. Easier said than done, but it’s a step in the right direction. 💪🏻

Nice result of the biopsy. I think is not so rare case. I mean fibroscan shows F3/F4 and biopsy F1. I think is more common than decreasing fibrosis from F3/F4 to F1/F0 in one year (based only on fibroscan results) for example.

Of course fibroscan is a good tool if you know its limitations (false positive rate, the prevalence and so on). And biopsy is not recommended in all cases.

I have some impression that sometime even specialists trust fibroscan results too easy.

That is wonderful news. Same thing happened to me, I suggest a biopsy if your numbers are high. I was diagnosed with 'probable' F4 cirrhosis by fibroscan but biopsy Stage 1 fibrosis zone 3 w/mild fatty liver no inflammation. My gastroligist was ready to settle with fibroscan results then primary referred me to hepotoligist who doesn't do or agree with fibroscan and he did the biopsy. So glad he did! Good luck..

Good news and it is probably safe to say that you don't have cirrhosis. It is important to take the fibrosis very seriously. Early stage is quite manageable for most people but it does require paying attention to the lifestyle changes and accept that it is not a short term thing.

Yes indeed and thank you! I will do all I can to change my eating lifestyle and exercise. My job is mostly sedentary, but I’m sure I can find ways around that. I have no choice but to lose the weight. I also have a hiatal hernia, degenerative disc disease with spinal fusions, along with NASH. Hernia repair is tricky if I can’t lose weight as my stomach could tear sutures attached to the esophagus. No weight loss, no surgery. I definitely need to get ahead and not discount anything.

Worth noting that Fibroscan would not happen without some abnormal liver bloodwork. So there are not just 2 data points, Fibroscan and biopsy. The VA hepatologist said Fibroscan is about 95% of tests now. Only patients extremely obese for whom a valid Fibroscan measurement is not possible might get biopsies -- or if there is viral hepatitis or neoplasms. Biopsies are rare, in her experience.

That means the VA depends on Fibroscan. Hard to argue with that.

There is a third Fibroscan reading. CAP, Fibrosis and also quality of measurement. Was your measurement quality poor? That may have generated the biopsy.

Indeed, sometimes some other factors like abnormal imaging studies, symptoms, family diseae history or just simple fatty liver are enough to consider doing fibroscan.

If we do it as screening test (without any indicators) we should be aware about high rate of false positive results (not because of quality of FibroScan but the prevalence of liver disease in that case). More below.

Fibroscan's have a tendency to overestimate when there is active inflammation. My initial fibroscan (with elevated enzymes) showed F3, nearly F4, then my subsequent fibroscan 6 months later after I'd got my enzymes down was F2, nearly F1.
