Regarding diabetic foot ulcer issue.

Hi Doctor, My father Age-53,weight-39 and he is having diabetic since last 16 years and he is suffering from diabetic foot ulcer from last 4 years on right leg.He is undergoing alternative dressing with normal saline and BioFil-AB particles using wound treatment(and also used hylogel ointment for some period).But doctor since last three years there is no major wound healing its improving very slow and tissues also not developing around that ulcer.He is always having some or the other ulcer over the foot & the foot was mostly in dressings for these reasons. My Fathers Details are as follow=: Age-55 Weight-39 Sugar-: 128 after lunch BP-: Normal Thickness of wound-: approximately 3mm

Ointment used for dressing-:we used till date hylogel and BioFil-AB and betdine powder particles with normal saline

Can anyone suggest best wound management treatment so that it heals fast.

Prasant, I am very much distressed to see and read what you have written. I am sure you must be caring for your father.

It is quite obvious that your father is grossly under weight, he may also be malnourished and most probably bed ridden. I am shocked. He is only 53. The wound or ulcer on the leg is not good looking. There is swelling of leg as well as redness around it, it means only one thing that it is infected.

As you have not mentioned any details about blood sugar , any medication or his diet, I am suggesting , no, I am requesting you to do a few things. Please consult a good diabetologist ,get him investigated in details about diabetes and all related things. Also reorganise his dietary schedule. Consult a senior and very experienced surgeon for his ulcer. It is apparent that he needs higher antibiotic not oral but injectable .

Now about ulcer and overall care of your father. Some one in your house must take his full responsibility,right from diet to dressing and day to day care. If you seriously ready to do it ,I can guide you how to do dressing.

Please take a very very good care of your father, he is suffering,please give him some peace . If you sincerely take care of your father, The GOD will take care of you.

Thanku sir for u r valuable inputs.for your info his sugar level is under control and regarding diet no rice only chapati and sabji. can you please guide how to do dressing with some good ointment?

Before I talk about dressing,please understand that without improving nutritional status,including any anaemia if he has any,and controlling infection if it is there ,it is not possible to improve the ulcer. It is not an easy job and it will time,so patience must be exercised.

Now, the materials that is required for dressing cotton, gauges,bandages, betadine ointment , betadine liquid, EC liquid ( electrolytic chlorine), gentian violet crude crystals. Please make a concentrated solution of GV.

Please take some warm water and with the of some cloth give hot compression around the ulcer for a 5/10 mins. Caution,water shouldn't be hot, as will damage skin. Now soak a gauge in EC, and cover the ulcer and let it remain for some time. Please remember the white which is seen in the ulcer has to removed,slowly and gradually. Remove the gauge and pc,ran the whole ulcer with liq.betadine.dry it and paint ulcer and the skin around it with GV solution. After it is dry put betadine ointment all over copiously and cover it with gauge piece over it put some cotton and bandage it lightly. You have to do it twice a day.

Probably,the best treatment for this problem is available in Chennai with M V Hospital For Diabetes.Just search for more information on the net.It is a rare case and there may not be many people knowledgeable about the problem.My sympathies are with you and earnestly hope that your father will become healthy again.
