I am a 37 years old man, having from 3 months ago digestive issues:
- following a parvovirus b19 infection I experience altered bowel habits - loose stools (not necessary waterly), no more then 3 per day.
- i have also good days, usually is like a pattern 1-2 days with almost normal stool, 2-3 days with soft stools, close to waterly, no more then 3 per day.
- no diagnostic so far, the gastroenterologist keeps say that is a un un-balanced gut.
Investigations:
Calprotectin levels 24 in july, 498(!) in late august, 198 in end of September. CRP, ESR levels normal. Colonoscopy when calprotectin was 498 was normal. Upper Endoscopy normal. Regular CT scan (no contrast substance) normal. pANCA negative, ASCA IGG negative, ASCA IGA borderline. lactoferrine 8 (cutoff 7.4). Occult hemorrhage negative. Stool culture (extended) negative. Parasites negative. Stool examination negative for leukocytes. Gluten tests negative. Genetic positive homozygous for lactose intolerance (C/C genotype)
So, except my calprotectin and lactoferrine, ASCA borderline, nothing is wrong.
Symptoms are still present, some days are good, some are bad but again no more then 3 stools per day max.
I have tried almost all probiotics known (really, including VSL#3), nothing seems to work.
Any experience for high calprotectin, normal esr/crp, no fever, normal colonoscopy and endoscopy??
Many thanks!
Andrei

Hello there, did they take biopsies when you had a colonoscopy? Microscopic colitis is often not picked up with a colonoscopy, it is diagnosed usually through biopsies. Although it normally causes chronic diarhoea. You don't say whether you have any pain. All the tests you have had done are not pointing towards anything sinister, so maybe (fingers crossed) if you look after yourself it may clear up on it's own. wishing you all the best.
Thanks for your answer, no pain (maybe some mild, very mild discomfort of left(!) side, around splenic angle). No biopsy taken due to lack of any modification on macro level(!!!). So, up to this moment only calprotectin and lactoferin together with this mild diarhoea.
I had calprotectin levels of nearly 500 also and I had two blood tests one week was “normal” then the next week when I was obviously flaring I had high CPR and ESR levels.
My colonoscopy was clear and I was actually diagnosed with Crohn’s with an MRI enterography my Crohn’s is located in my small bowel.
I didn’t have typical Crohn’s symptoms my doctor told me as I wasn’t having enough bowel movements to have Crohn’s which is complete and utter nonsense.
I got fobbed off for years being told I probably had ibs.
