Yes I use one and it has a velcro flap to put the wire through so it doesnt flop around. I dont have a problem with it.
I use in-ear headphones that clip over the ears and they have never fallen out or off! Not sure of the brand but think they could be JVC - got them in Tesco's!
I use my 'phone and keep it in the zipped pocket on my jacket, I then clip the lead to my jacket. This last trick I learned after my very first run as the lead left hanging dragged the earpiece out, since I've done this they've been no problem at all.
Got an armband thingy at lunchtime, was thinking of wrapping the long headphones cord around it somehow, or clip it somewhere, as Fingalo suggests above...will experiment tonight...!
I only put one earpiece in when I'm out and about running so that I can hear the traffic etc, the other earbud and wire I loop up and tuck into the armpouch. Works well.
Am hoping for a specialist 'one eared earbud' for Christmas which will save me he hassle of looping up the spare wire. (If i get out my phone to check times etc then if i have a jacket over the top it can be a pain to get the spare cables back in). I think they are called 'One Good Ear' and are designed for running.
Update - had every intention of testing the pouch armband thingy out last night...got home, got ready, child was kicking off, I got stroppy, toys were thrown out of pram (by me), armband was launched across room.
Will try it another night ;o)

I use Sennheiser in-ear headphones that isolate, but not for road use. These are great for off road runs, but are a seriously bad idea for road running, even at night when you can see headlights.
There are some new(?) 'phones out that don't use your ears (witchcraft if you ask me) that are safe for road use, or just play safe and don't listen to music when on the road.
Got some in-ear/over ear hooks type Sennheiser/Adidas's yesterday, and whilst they will never be as good sound quality/bass as my normal over ear (cans) type, they are still pretty decent.
