Really wasn't in the mood for jogging today so decided not to attempt the 3rd run of week 5. Glad I repeated run 2 as the last 4 mins were mentally tough. Managed just over 4km in the total time. Do others measure with or without the warm up and cool down time and distance?

If you want to know how much ground you cover in a workout including warm up, then count them in. If you only want to know what you cover in the run/walk intervals (or runs, in the coming weeks) then start your tracking when your warm up finishes.
I should be doing Run 3 of Week 5 on Thurs but may repeat Run 2 once more first. I never measure the distance of runs but it would be interesting to start. Good luck for Thurs!
