So i did my first run in 3 months and i started at w1r1 fully believing it wouldn't be as bad the second time round. It was, there is no way i can skip to where i was before (w4r3) so i guess im starting from the start. But it feels great to be back and running outside instead of at the gym. I loved it.

Well done for getting back on it. 3 months is a long break, but you have the benefit this time around of knowing you really can do it. Good luck to you.
Just take it slow it can take up to six months for a really bad sprained ankle to heal talking from experience. Did mine one time when down the Falklands and had to walk on it for two months came back and had to do a fitness test and managed but nearly passed out after the last shuttle with the pain. Going back to W1 might be a good idea just be prepared to stop if it starts to hurt.
Hey the fitness just went awol for a while, hardly surprising, but I think your stamina, km bank and muscle memory should hopefully make it a bit easier this time round. Oh and you've done these runs before so in your head, you're ahead. And that is never a bad thing!
