Is this anxiety playing up again?

Hi, I have not been on here in a while because things were getting better until yesterday I had these weird symptoms in my chest and neck to my head. These symptoms in my chest felt like air was being punched out through my chest or like something like my soul was coming out of my chest it sound silly but that's what it felt like, then I get these split second dizzy spells in my head. Going through all that last nigh I could not sleep because whilst I was dosing of it felt like I was dying and I would quickly wake up panicking and being scared to sleep, this happen once in a while but the symptoms are always there. Is this all down to anxiety or should I be worried. Many thanks
