Risk of harm/ medical record

Hi all

Just a the quick one, i just recently went for coucilling service where they informed that everything is confidencial but ill have to sign a form which is risk of harm etc, the problem is i am a new starter with myemployment and im worried that will go against my medical record!? Its doesnt sound good if your company would find out .. please advise

Do i really have to sign anything?
