PSA after RP.

Hi all,

I had my first blood test today, 10 weeks since RP. It was the standard PSA test and not the ultra sensitive test. The results were <0.1

I guess that means it’s between 0.00 and 0.09.

That’s good right?

I have a ultra sensitive PSA test on March 1st.

Where do I want that # to be?

The technique they used to measure your PSA does not detect less than 0.1, so with this technique PSA is undetectable.

The ultrasensitive PSA test is useful to follow PSA after the prostatectomy to detect early if the PSA starts going up. This helps to do early salvage radiotherapy if it were necessary.

The phycology of watching the Psa is a bad one . It’s kind of like we predict a rise on ourselves because we are told that #4 pc has no cure and always reoccurs if given time . Your Psa hopefully will never go up again!With an RP if they got it all you could be cured ? If it’s not going up then just enjoy that for all it’s worth . One day not worrying about the Psa is a good day . It wasn’t until my fifth year of being detectable that I’d even mention it .I Didnt want to jink it . Good luck Sir! 💪

You can get uPSA tests on your own from online labs like this one for about $50 after discounts. They issue a req and the draw is done at Quest/Labcorp.

It’s perfect . An ultra sensitive test can possibly drive anxiety with minute changes . I demanded those my first years then decided <.1 is good enough for me .. I too was Psa< .1 and I’ve remained so for almost 8 yrs now . Keep it rolling .

Here are a couple of good articles on uPSA (ultra low)...my Drs have blown this data off but I think its a good way to manage your PCa and should provide predictive value. If PSA never comes back great...but if you sense that its coming back it provides you time to make better decisions...what we dont want and cant manage with PCa is waiting until the symptoms drive us to action...it may be too late then for many treatments. PS also look at this great mapping of all the genomic tests out there...dont be shy about asking or these. Again, they provide predictive value, give peace of mind in how you manage your desease and may allow better decisions on future treatment.

There are two classes of precision for the so called ultra sensitive test: Reporting to the second vs to the third decimal place.

Assessment for the 0.0x class:

0.01 or less -> Excellent prognosis

0.02 -> Quite common, should be en guard for the future though

0.03 -> Recurrence probability during the following 2-3 years 50-50%

0.05 and up -> Dubbed "Persistent PSA" i.e. more treatment to follow after surgery healing
