It’s been a few months since I’ve last posted (had a baby in October and my dad’s PCa was stable-ish) but it looks like the most recent scans showed growth. He’s 71, dx’ed nearly 3 years ago, and has already utilized Zoladex, docetaxel, and Zytiga and some targeted radiation. He’s had widespread mets since dx but largely feels pretty well (aside from Covid knocking him on his rear in October).
With Zytiga losing efficacy, we have choices ahead of us: Xofigo, Jevtana, or three different clinical trials. I feel knowledgeable about the risks and benefits of Xofigo and Jevtana but I’ve never seen these trials. Would love some input on them if you’ve got some expertise to lend!!
1. if it helps, VEGF inhibitors haven't been effective against PC2. Many growth factor inhibitors have failed as monotherapies. If they work, it may be only in combination
3. Degrading the androgen receptor may make sense if you have overexpression (likely if you are CR). You can check with a biopsy and IHC for the AR before you try it.
Thank you, Tall Allen. Yes, a blood test is in order for ARV-110. So far, that’s the one I’m in favor of, if we go with a trial. We may just stick with Xofigo or Jevtana.
I would consider the ARV 110 trial. It is not a randomized trial, he will receive the treatment and there are preliminary data showing that it can work in some patients.
What worries me about the ARV-110 is that it is not effective for the common and problematic ARV-7 variant. Any additional way to assess for that and address it?
