I was started on hormone therapy last fall after exhausting all other (USA) treatment options. Initially I had monthly Firmagon shots in my belly (ouch!) and both my PSA and testosterone went close to zero. After two months my MO switched me to Lupron. Since then my PSA has been steadily rising. I wonder should I go back to Firmagon?

If you do not mind the monthly injections you can. Your urologist would probably rather take Zoladex or Trelstar instead.
Check you testosterone level. It should drop below 0.7 nmol/L. Sometimes this does not work with Lupron and other drugs.
Is Lupron keeping your testosterone level low? If not, try Firmagon or add Casodex. If PSA is rising in spite of low testosterone, and you don't have detectable metastases, you might qualify for Erleada or Xtandi.
