As a newbie, first lot of bloods since I started Levo 6 weeks ago

These are private bloods as NHS only does FT4, TSH and Antibodies only if you jump through hoops! Please advise. Still have intrusive muscle and tendon pain and so hope I will get an increase in Levo. Also should I have requested others?

Free thyroxine (FT4) 17.8 12.0 - 22.0 pmol/L –

Thyroid-stimulating hormone (TSH) 5.16 0.27 - 4.2 mIU/L –

Thyroglobulin antibodies (TgAB) 16.2 0.0 - 115.0 kIU/L –

Thyroid peroxidase antibodies (TPOAb) 14.8 0.0 - 34.0 kIU/L –

Thyroxine (T4) 110.0 59.0 - 154.0 nmol/L

Active B12 >150.0 70.0 - 150. pmol/L - Is that OK?

Folate 32.0 10.0 - 45.0 nmol/L

Ferritin 76.5 30.0 - 200.0 ug/L

hi Lindy,

It looks to me like you are due an increase based on that over range TSH.

What dose are you on and how long has it been?

When increasing do it slowly, 6-8 weeks and test again

Try and stay on the same brand Levo (you can ask the pharmacy when you pick up and they should help you out. It’s a government guideline).

B12 is nothing to worry about, being very high is good for hypos, some of us don’t actually absorb what we have circulating anyway so it can be misleading and we pee out what we don’t store and recycle, so again being over isn’t a concern.

I also test for vitamin D when I’m doing a panel like this.

Thanks fir the reply. I have had 6 weeks of 25mg. Most of my vits and mins are Betterme sprays as they absorb better and aren't diverted by low stomach acid. I agree and willing to do battle with a very reasonable GP if necessary!

was test done as recommended, early morning, ideally before 9am and last dose levothyroxine 24 hours before test

Which brand Levo are you taking

See GP for next increase to 50mcg daily

TSH is still too high

Free T4 (fT4) 17.8 pmol/L (12 - 22) 58.0%

Free T3 (fT3) 3.4 pmol/L (3.1 - 6.8) 8.1%

Initially you may be better to increase to 37.5mcg daily for first few weeks before next increase

No vitamin D result?

Test done at home fasted at 8 30am. Thought I had ordered Vit D but clearly didn't! Teva brand. Speaking to GP next Wednesday so not much time to change. Thank you for the response.
