I've now received my next prescription of ndt my first prescription was 0.05 grains, I increased to one and a quarter grains which showed an improvement in my thyroid hormones, still my tsh and t4 need an increase my t3 is 4.3 in range.
I have received one grain capsules now and I just wondered if anyone knows the ratio of t4, t3 in one grain🤔 there's no info on the paperwork that came with the new prescription.
PM if applicable 👍

I'm just trying to workout how I go about splitting the one grain capsules ( I take the powder out and split into 3 amounts)
I've to increase to one and a half grains which I'm now doing then build up to 2 grains....but on the 7th of September 3 days before my blood test started to feel unwell, I was supposed to have thyroid bloods done as I'd been on one and a quarter grains for around 10wks...the nurse did the wrong blood test she did parathyroid instead of thyroid..but my parathyroid was over range ( I have primary hyperparathyroidism )
I still feel unwell it's like I've gone backwards 😔 I was doing so well, although I feel my symptoms are hyperparathyroidism and not the ndt..I did think perhaps I'd over medicated as I've started to sweat and shake, but I get those when my calcium goes over range, so it's really hard to distinguish between over medicating and primary hyperparathyroidism as id been on 0.05 grain capsules i thought the one grain may contain a higher dose 🤦♀️
Sorry you are still not quite right birkie , but maybe it is your parathyroidism that is playing up and confusing things. I still wouldn't split up a capsule and just get smaller doses made up by Roseway. They do go right down to 1/8th of a grain.
The reason I take the powder out is because of the capsules themselves, I've had bad stomach problems taking medication in capsule form, so the clinician told me to eliminate anything that might upset my stomach, so take the powder out .We'd rather start you on ndt with no obstacles (ie the capsules) and I've had no gut issues since taking the ndt🙌 and yes I agree, I'm sure my flare ups are parathyroid, as my parathyroid as gone over again 😔 I appear to be coming out of it as I'm feeling a bit better these last couple of days.
Ho blimey, I have really bad heartburn and Acid reflux when I take any capsules 🤢 my prescription is 1 grain atm I'm splitting the other 1 grain in half to add to the other one grain, making 1 and a half grains, then I split it into 3 morning, afternoon, evening. I couldn't stand taking my T4 or T3 in one go, I always felt over medicated so was advised to split my meds in order for my body to adjust to them, so far it's working 👍 but my next bloods in January will be interesting 🤔
That's a better idea 👍 my next prescription is Jan but I may be up to 2 grains by then🙏 so 2 grain capsules would be better that the one grain , I could then split the one capsule in to 3👍 I'll need to mention it to the clinician in January [re - my blood results]😍
OK, have realised that you must be on compounded NDT from Roseway. So unless there’s someone else here who takes it, I don’t think we can advise on how much T3 and T4 is in it. I went to the Roseway site but it doesn’t say. Could you ask them perhaps?
Whatever it is, their half grain capsules should contain half as much T3 and T4 as they provide in their one grain capsules.
I guess the content has to be Thyroid USP powder - and believe the T3/T4 content in all NDT brands to be 1/4.22 - T3/T4 - with the difference being the fillers used to make the powder into a tablets.
I shouldn't worry about your TSH responding - with Graves the TSH may never recover -
and without a thyroid - your HPT axis - the Hypothalamus - Pituitary - Thyroid feedback loop has been down regulated as you haven't a thyroid in situ to complete this circuit loop.
The leading brand of NDT is Armour and states content of 1 grain as being - trace elements of T1, T2 and calcitonin plus 9mcg T3 + 38 mcg T4 per grain.
So I would imagine 1 + 1/2 grains will be around 13.50 T3 + 57 mcg T4 -
with 2 grains being around 18 mg T3 + 76 mcg T4 :
I've been settled on 1 + 1/2 grains for 6 years now and with Graves post RAI thyroid ablation 2005 and dose once a day.
As I am taking the same as you birkie , the information on my bottles of NDT says each 1 grain capsule contains 38 T4 and 9 T3. I don't think you can really divide NDT capsules, but Roseway Labs will put the exact amount you need in each capsule. For instance, my capsules contain 0.625 grains, 0.5 grains and 0.5 grains. for each 24 hours. I take mine at 8am, 3.30 pm and 11 pm. I find them very stable. The prescriptions have to be written for same. I hope you get on with them alright.
I do not use the Roseway Prescriber, but a private one. I find this better.
There's no information on my bottle, only 1 grain of natural dessicated thyroid, the clinician told me to take the powder out of the capsules as they contain gelatin, she said I could split the compound that way i dont know how to get roseway ndt anyother way than using them.
It's working I can have the 1 grain, then split the other to make up 1 and a half grains, it's just ive gone backwards in health since Sep 9th ..symptoms don't appear to be anything to do with the ndt though, my bloods showed my parathyroid hormone was over range [ I have primary hyperparathyroidism]
I've just woke up 😴 I'm struggling to get up in the morning and sleeping in the afternoon, I feel wrecked...but as I say I think it's primary hyperparathyroidism also my kidney function was abysmal, and I've got right loin pain😖 drinking plenty tho as we are told to do with primary hyperparathyroidism 🙄
I was led to believe they could not be split as you might get all excipients in one half and all NDT in the other half. Roseway can put what amount you want in a capsule. I think they said this could be as small as 1/8th grain (0.125). I also take mine out of the capsule and dissolve in water.
That's interesting, I may need to put mine in water , but the clinician said I've to go slowly with it.. and I already split T3 because I felt over medicated taking the full amount, so she told me to split the powder into 3 ..so 1 and a quarter grains split in to 3 ..I could try taking the one grain but would still need to split the other for the half..[ 1 and a half grains]
As I say I'm pretty sure I'm feeling rotten because my parathyroid hormone as gone overactive and by now I reckon my calcium as followed suit🙄
HiI started very slowly on half a grain, on July 18th this year I tried the synthetic form of T4 ' T3 for 5 years but it gave my terrible gut issues in the end I wasn't absorbing enough to move my T4 or T3.
Moved on to 1 and a quarter grains of ndt my T3 is now in range although my TSH still as a way to go as it was 45.66..it's now 18.3 T4 is 8.4... so I'm now on 1 and a half grains, I'm slowly moving to 2 grains , but so far I've had no side effects , re do my bloods in early Jan on 2 grains, be very interesting to see the results 👍
I don't think I can help you much RoseStar , as I changed straight from another make of NDT. Even when I went over to NDT, I changed from T4 plus T3, so my transition was almost there and I did not have any trouble. Roseway NDT is quite good and gives me good blood levels and stays quite stable, but I still think WP Thyroid is the best, but, unfortunately, no longer on the market. I wish you good luck with it.
