What on earth is going on with my T4?

I've just got my latest test results back. I am currently on100mcg levothyroxine which was raised from 75 in June. However my T4 levels have significantly decreased since June despite the rise in levo. My TSH results have also decreased. Can anyone explain what is likely to be going on?

I've also had a T3 test done privately this week by Monitor My Health and the result has come back as 3.8 pmol/L (range 3.1-6.8)

13 November 2023 3.169 mU/L

21 September 2023 1.841 mU/L

25 July 2023 3.730 mU/L

T4 results Normal range: 7.9 to 14.0

10 September 2024 6.7 pmol/L

3 June 2024 7.4 pmol/L

10 February 2024 8.6 pmol/L

13 November 2023 8.6 pmol/L

21 September 2023 8.7 pmol/L

25 July 2023 7.8 pmol/L

3 June 2023 7.4 pmol/L

I was supposed to have a ferritin test as well, but they forgot to add it. My ferritin was 53 in June and I'm following the Iron Protocol and have raised it from 14 last November.

I also inject 1500 mg B12 every other day as I was right on the bottom end of normal last year and suffering quite severe neurological and physical effects plus folate, magnesium 8000iu vitamin D3\K2 and eat as much potassium rich food as I can. Not had these tested since starting supplements.

Was test done early morning and last dose levothyroxine 24 hours before test

Do you always get same brand levothyroxine at each prescription

Your results show you need next increase in Levo to 112.5mcg daily

Retest in another 8 weeks

Meanwhile recommend you test vitamin levels, especially vitamin D

That’s quite a high dose vitamin D

full iron panel test

retest 3-4 times a year if self supplementing.

It’s possible to have low ferritin but high iron

Test early morning, only water to drink between waking and test. Avoid high iron rich dinner night before test

If taking any iron supplements stop 5-7 days before testing

Thanks for replying. My test was done at 8am and I had fasted since the previous evening and just had a glass of water before my test. I had not taken levothyroxine in the 24 hours beforehand.

The brand of levothyroxine I've been taking has been the same (Accord) since I was started on 50mcg last year.

I'll look into getting the iron and vit d tests done. Last time I had it done my iron transferrin was low (21.5%, normal range 22-36) and my serum iron was 14 (normal range 2-32).
