Hi it's me again but this time I'm looking for advice for my daughter who has had Hypothyroidism since a teenager. Now 45. I've told her she needs to increase her thyroxine but thought I would ask the experts. They have never tested her T4 or T3. Her vitamins were tested last in October 2022 which were dreadful & I bought her a B12 spray as she forgets to take or is reluctant to take tablets.
Her results are all over the place & she has had them all early morning but I doubt she omitted her thyroxine the night prior to the tests as she wouldn't have know about that. (neither did I till I asked this group). Here are her results.
2018 TSH 6.1
2019 TSH 0.05
2020 TSH 0.13 Tested 2 months later TSH 0.05
2021 TSH 0.07 Tested 2 months later TSH 5.4 then again 4 months later TSH 1.1
2022 TSH 0.48 Her vitamins were B12 290, Ferritin 24, Folate 92, & Vit D 39
2023 TSH 4.0
She currently takes 100mcg thyroxine x 4 weekly & 75mcg x 3 weekly
As you can see her Vitamin levels are very low but her GP has done nothing about any of it.
Next steps advice please

2022 too long ago now to advise on vitamins but clearly terrible
Just testing TSH completely inadequate…..but TSH over 2 is too high
Suggest she either
push to increase dose to 100mcg daily now and then get full thyroid and vitamin tests in 6-8 weeks
Or
Retest full thyroid and vitamins now
Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test
This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)
Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

Disgraceful that your Daughter has been hypo since a teenager and T4 levels have not been tested.Best to do private testing as the GPs won't test T3.Id be on with surgery first thing in morning and insisting on 100mcg daily for two months then take the private test.I don't know of many people who are doing well on 89mcg daily.
