Increase levothyroxine or add liothyronine

Results on 75 levothyroxine

TSH 0.84(0.4-3.7)

Ft3 3.6 (3.5-6.5)Ft4 14 (12-22)

Would you increase levo to 87,5 or add 5 mcg liothyronine? Which would make most difference to TSH do you think 5 mcg T3 or 12.5 T4?

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

is this how you do your test

Ft4 is too low, so increase levothyroxine

How much levothyroxine are you currently taking and how long on this dose

Which brand

Approximately how much do you weigh in kilo

When were vitamin D, folate, ferritin and B12 levels last tested

Yes that's how I did test. Been on this dose for 2 months. Weigh 63kg. Am supplementing VitD and iron because of suboptimal levoels of those.

63kg x 1.6mcg = 100mcg levothyroxine as likely daily dose levothyroxine required

So increase to 87.5mcg and retest again in another 6-8 weeks

Meanwhile continue working on improving low vitamin levels

We need optimal vitamin D, folate, ferritin and B12

What were last vitamin results

Too soon to consider T3.....it's not the panacea of all ills. And that's from someone who needs high dose T3-only to function!

TSH isn't the most important result tho' ill informed medics wrongly think it is

FT3 is most important followed by FT4....and yours are both too low

For good health every cell in the body needs to be flooded with T3 by way of an adequate and constant supply.

With FT3 so miserably low you must be feeling underpar.

Your T4 to T3 conversion appears ok but the test results after the increase will help confirm this

You are undermedicated!

With your labs I'd raise levo to 100mcg and test again in 6+ weeks....you have plenty room to safely increase.

Important that you optimise vit D, vit B12, folate and ferritin....with your results I'd be fairly sure you need to work on these. Any recent results?

Good luck!

So not long stopped T3

Highly likely vitamin levels are low

So next step test Vitamin D, folate, ferritin and B12
