If your TSH was over 30 at 4 pm, I dread to think what it would be at 9 am! With a TSH at that level, it doesn't really matter what time you do the blood draw.

Considering your dose, your TSH is very high, and your Frees are very low. Did you leave a gap of just 24 hours between your last dose of levo and the blood draw? If so, then can you tell us how you take your levo? Do you take it with food, or other medications/supplements?

the only thing different with this blood test was the timing , i didnt take levo that morning in case they did a draw . i told the Dr i had increased my dose by 25mg after the last test , it didnt seem to register with her .

good 1 -1 /2 hours between levo and food . i have stopped the b12 & d3 i was taking for now .

im feeling really overwhelmed by it all , feeling like i just want to withdraw into myself and not engage with anything .

So, there was a gap of more than 24 hours between the last dose of levo and the blood draw, if the blood draw was at 4 pm? If so, that would account for the low FT4 - to a certain extent. And, the low FT3 could be due to poor conversion.

So, that leaves the high TSH. What was your last TSH before this one?

Sept 2021

this test was again afternoon and had taken dose about 7 hrs before 225 mg

TSH 5.96 ( 0.35 - 5.5)

T4 19 ( 9.0 - 23)

i know timmings off but it was during pandemic and you got what you could .

was told just depression and offerd therapy . I was not depressed until after i spoke to the Dr . partly the reason it has taken this long to go back . symptoms have snowballed from that point .

OK Well, the timing of your dose doesn't affect your TSH. It only affects your FT4. So, you have a false 'high' FT4 there.

It's the TSH that is affected by the time of day - if only doctors knew that! But, even in the afternoon your TSH was far too high, especially for someone on thyroid hormone replacement. However, not as high as your last TSH! So, that suggests interferrence in the assay - could be antimouse antibodies affecting the method of analysis. So, what is needed is a new test in a different laboratory. Which should either prove it's a true reading, or disprove it. But highly unlikely that your TSH could have shot up that high given the dose of levo you're taking, even though your Frees are low, suggesting poor absorption.

only did repeat thyoid test but at least they did T3

i have emaild for the list .

fogot to mention the ferritin at the appointment , i felt she was uniterested enough with the thyroid . I was going to take a list in with me with questions but a friend who is a nurse said not to as Drs apparently hate this .

Ft4 false low then …as last dose would have been more like 36 hours before test

So Ft4 may be improving

Retest again after 6-8 weeks

High TSH could be due to mouse 🐁 antibodies

a short list is fine

And (even today) they take more notice if you take a supportive MALE along to the consultation…..assuming it’s a face to face appointment
