My thyroid battle.

I have posted on here a few times since xmas about my battle to get my hypothyroidism taken more seriously by my doctor.

Due to my surgrey having major staffing issues( they turned E-Consult off for a week) I decided to switch surgeries to another one in town which my partner was registered at.

Once registered there I finally got an appointment to discuss my thyroid on 2nd May. She seemed really nice and listened to my concerns that I fely my hypothyroidism had been neglected by previous doctors since being diagnosed in 2016 and that Metformin may be suppressing my TSH.

To that end she agreed to order a full thyroid screening panel and to check my HbA1c so that it was still in the mid 40's and not above 50 she would let me stop the Metformin for 2 months before rechecking my HbA1c.

As I live virtually next door to the local hospital I booked my bloods to be done there at 0820 the following week.

On the day I checked the blood forms and she was good to her word. Thyroid screening, Cholesterol, HbA1c and Thyroid Peroxidase antibody were all there.

I finally got the to she the results yesterday and good news/bad news

HbA1c 43 mmol/mol

Range (0 - 42)

TSH 1.72 mU/l

Range (0.34 - 5.6) 26.24%

FT4 13.5 pmol/l

Range (7.7 - 15.1) 78.38%

No FT3 or Thyroid antibodies although I am not supriused.

So I am going to make an appointment to discuss these results and hopefully come off the Metformin for 2 months as we had previously agreed, but looking at that FT4 result I am not sure what she is going to say.

Any thoughts?

Very odd ranges

Was last dose levothyroxine 24 hours before test

Suggest you get TSH, Ft4 and Ft3 and antibodies tested now

Also vitamins retested via GP or include on private test

2 months ago all four vitamins were terrible (almost certainly as result of being hypothyroid)

Vitamin D 35nmol - optimal is at least over 80nmol

Serum B12 249 - very low - optimal over 500

Folate 10 - aiming for top of range

Ferritin very low at 58 - aiming for at least over 100

couldn’t see any thyroid antibodies test results in previous posts

Have you ever had antibodies tested or ultrasound scan of thyroid

all thyroid blood tests early morning, ideally just before 9am and last dose levothyroxine 24 hours before test 

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

No I have never had my antibodies tested hence why I asked the doctor to do them.

As for the vitamins I have found it a bit confusing to be honest. Did speak to the doctor about it and she said my B12 was probably low because of the time of year ie just turning to spring.

I have started eating pate 4 or 5 times a week and brought some max strength vitamin d from the supermarket and have started taking those.

As for the other’s money is a bit tight at the moment so struggling to justify it to the missus.

Levothyroxine can’t work unless vitamin levels are at GOOD Levels

What dose are vitamin D tablets

Vitamin D tablets must be taken at least 4 hours away from levothyroxine

GP should prescribe 1600iu everyday for 6 months

NHS Guidelines on dose vitamin D required

GP will often only prescribe to bring vitamin D levels to 50nmol. Some areas will prescribe to bring levels to 75nmol or even 80nmol

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

You/she meant vitamin D

Many (most) thyroid patients need to supplement vitamin D continuously.

You might need higher dose in winter than summer

Also worth getting your wife/family vitamin D tested

on levothyroxine we frequently need to supplement B vitamins

A separate B12 initially and a separate vitamin B complex

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and add a separate vitamin B Complex after a week or so

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12

Only add one supplement at a time then wait 10-14 days to assess any changes before adding another

75ug Vit d is a minuscule dose.

I take 3,000 iu summer & 6,000 winter. Go and get a good dose Vit D 3 and start it asap

I think I might be missing what you are saying here.

75 ug (75 μg or 75 micrograms) of vitamin D is 3000 International Units. So exactly what you take in the summer.
