hi everyone,

Before my Hashimotos was diagnosed about 7 years ago I had started to suffer with oesophagitis. As soon as I was put on T4 this completely stopped. It’s now back with a vengeance - I was awake for ages last night with very bad chest pain - in the end drinking water eased it. If I eat dry food quickly it gets stuck too.

Does anyone else suffer with this and what has been found to help. Does it mean I’m not taking enough T4/T3. I take 50 mcg T4 and 15 mcg T3 daily. My latest results are:

free T3 5.6 (3.1-6.8)

Free T4 8.8 (12.0-22.0)

TSH 0.03 (0.27-4.2)

any advice as usual will be greatly appreciated

ALWAYS test early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

T3 ….day before test split T3 as 2 or 3 smaller doses spread through the day, with last dose approximately 8-12 hours before test

is this how you did this test

Do you always get same brand levothyroxine and T3 at each prescription

Which brands?

How much levothyroxine were you taking BEFORE T3 was added

Many, many members on levothyroxine plus small doses of T3 find they need BOTH Ft4 and Ft3 at least 50-60% through range

suggest you request 25mcg increase in Levo to 75mcg daily

Retest in 6-8 weeks

What vitamin supplements are you taking

When were vitamin D, folate, B12 and ferritin levels last tested

Hi slow dragon, I follow the protocol for blood tests and it was 8.30 in the morning.

I take T3 5mcg and T4 50 mcg at 6 am.

T3 5 mcg at 12 noon

T3 5 mcg at 6 pm

I was taking 100 mcg T4 before T3 was added - both Mercury Pharma

I’m taking B12 sublingual 3000 mcg Methylcobalamin/ Adenosylcobalamin

Magnesium complex 375 mg

D3 + MK-7 K2 3000 iu

Blood tests 4th Feb 2025

Serum vitamin B12 275 (197-771)

Serum D3 68.7 (over 50 sufficient for most people)

I haven’t got folate but these are iron results:

Serum iron 26.3 (5.83-34.5)

Serum transferrin level 2.07 (2.0-3.6)

Transferrin saturation index 56% (15-50)

Serum ferritin 245 ng/mL

Your B12 is extremely low for someone whose taking B12 supplement

Were you taking your B12 daily up to date before test?

Are you also taking a daily vitamin B complex

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose and may need 2 per day and/or may need separate methyl folate couple times a week

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 3-5 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12 if last test result serum B12 was below 500 or active B12 (private test) under 70

Increase to 75mcg daily and retest in 2-3 months

Presumably you are NOT taking any iron supplements

Are you post menopause?

Hi SlowDragon, thank you for all of this advice. I’ll go on the links shortly and read up. I’m not taking any iron and I’m 70 in November. I also have Sjögren’s with high antibodies.

hi SlowDragon, me again - I forgot to mention that over the past 18 months I’ve had a real problem with night sweats,plus in the day if I get overheated sweat pours from my forehead. I’ve never been a person who sweats a lot. I’m also waking at 4am and can’t get back to sleep

Night sweats are strongly linked to B12 deficiency

You may need B12 injections rather than B12 supplements

Ask GP to test for Pernicious Anaemia and request B12 injections to see if it helps

Explain you have been taking B12 supplements, but B12 levels are still low

Hi SlowDragon - I was tested for pernicious anaemia a couple of years ago - it was negative. I stopped taking B12 for about 4 months prior to blood test - only started again since the results. My GP said she’s not allowed to give me injections at the level on my bloods. Hopefully now I’m back on sublingual B12 it’ll help.

Did that also result in night sweats starting

important to take daily B complex as well

Hi, yes I’ve started Bcomplex today - can’t remember why I stopped but I’ve got them in already containing folate. I think I’ve had night sweats before stopping B12 but now I’m taking it regularly again I’ll keep a track of them- they don’t seem to be every night. My forehead sweats are a nuisance too as I have to have a fringe because of a scar from having skin cancer removed. My fringe drips. I thought I’d see how I go with sublingual before thinking about injections, last time I took it my bloods did rise to about 350. Thank you, I’ve never linked the sweats with B12 maybe it’ll improve now I’m taking it regularly now together with B complex

I wonder if there is a link between this and Hashi's? I had severe problems which were not diagnosed, turns out I had bile duct stones. I just wonder because the chest pains I had with it mimicked a heart attack, 3 ERCP's later and it still isn't properly sorted. I just think that with autoimmune problems the body does all sorts of strange things! (I also have Sjogren's). I also have a problem with food seemingly getting stuck.

Hi, I have Sjögren’s too but I’ve read that has Hashimotos also causes oesophagitis

It isn't a good combination is it? I just think there is so much unknown about these conditions and much more research is needed.

Yes you’re right - I wish we lived in a world where we could just put our health into the hands of drs and know that we’re being optimally treated. I don’t think it’ll ever happen

I have this and duodenitus. I have had these for over 20 years. I have found trying not to eat processed foods has helped. I sympathise with you as its horrible.

I take buscopan, gaviscon liquid which takes about 1 - 2 hours to sooth enough to go back to sleep. I am now on Esomeprazole 40mg daily which helps. I have had this for 30 plus years..... 😔

Oh no 30 plus years - I’ve had it about 10 but only just reared its ugly head again lately - it all stopped with T4 for about 6 years. As SlowDragon says I’ll increase my T4 and maybe it’ll help again. I’ll keep in mind your meds if not - thank you

Have you considered if you might have a food sensitivity? For example, gluten and dairy are common ones for Hashimoto's patients.
