please could l have some advice on my results done on 22nd of march 2023

Serum tsh level 0.09 range 0.38-5.33 Abnormal

Serum free t4 14. range 7.90-14.40 Abnormal

Dr contacted me to lower my eltroxin dose from 75 to 50 as my cholesterol was 6 and that could be the reason my cholesterol was high in the past my tsh levels have never been queried the blood test was done at 3.30pm as l was having asthma check and they said it wouldn’t matter having it later in the day, l had taken my eltroxin at my normal time at 7am

in august 2020 serum tsh level was 0.42 range 0.30- 5.00 this test was done at 9.0clock and since going on lower dose l feel cold and my breathing is,nt good even when l use my qvar inhaler which were the same symptoms among others that l had ten years earlier when diagnosed with under active thyroid , could it be the late time my blood test was done it has made a difference to my blood test results? hopefully someone can advise me.

Oh dear. You have a doctor who wants to dose you according to TSH. :(

For many of us who’ve been on thyroid hormone replacement for a while, TSH isn’t a good indicator of whether you’re on the right dose—it doesn’t always respond like it does in someone who doesn’t have hypothyroidism. It can stay suppressed, even when you’re undermedicated.

So your doctor shouldn’t be reducing your dosage. Your FT4 wasn’t over-range (it’s odd that it says it’s abnormal because it wasn’t). You weren’t over-medicated.

Finally, under-medication causes higher cholesterol, not over-medication. Your doctor has got that completely the wrong way around. :-o Doesn’t exactly inspire confidence, does it?

I’d ask to go back on the original dose. If the doctor you normally see won’t agree, see if you can see another one in the practice.

ah thank you jazzw , l thought l had read somewhere that high cholesterol was because patient is under medicated ,have been told to make another appt for blood test next month for thyroid and cholesterol so will see what happens , many thanks , Eve.

It's all about the FT3, which they don't even test! You're only over-medicated if your FT3 is well over-range. And, your cholesterol is high when your FT3 is low. So, the thing to do is to refuse to lower your dose unless they test your FT3. :)

thank you for your advice, why is it they don,t test FT3 when it looks like there may be a problem? it’s 6 weeks since l was told to cut dose to 50mcg and the only thing that has improved is my sleep, l,ve had under active for 13 years l,m 73 years old and my dose has been 75mcg for about 10 of those years. I suppose dr will be unwilling to test FT3 but l will ask , surely it would be better to test patients rather than than having them going back because they still feel unwell it doesn’t make sense 🤔. What would we do without this helpful website.

Inhaler probably contains steroids?

Steroids lower TSH

Suggest you get FULL Thyroid and vitamin testing including Ft3 and thyroid antibodies

ALWAYS test thyroid levels early morning and last dose levothyroxine 24 hours before test

Which brand of levothyroxine are you currently taking

When were vitamin D, folate, ferritin and B12 last tested

What vitamin supplements are you currently taking

Low vitamin levels tend to lower TSH

Low vitamin levels also increasingly common as we get older

Request GP test vitamin levels

thank you will mention about my steroid inhaler, to help lower my cholesterol l have been taking plant sterols ,salmon oil capsules, zinc CoEnymeQ10and magnesium plus a pro biotic and stopped taking vit D at the moment,(very expensive hobby) just hope it all helps . Had full blood test ferritin was 86, folate 8.5 B12 569 they have never tested vit D .
