Hi all, I am posting for a friend of mine who is not good with a computer, and for many years has not felt well or been able to lose weight. Any comments on these result?

She is currently on 75mg levothyroxine.

Free T3 203 (210 - 440)

Total T3 108 (76 - 181)

Reverse T3 18 (8 - 25)

TSH 1.35 (.4 - 4.5)

Free T4 1.2 (.8 - 1.8)

Ferritin, Folate, D and B12 all midway or better. It looks to me like she could use some T3. Would appreciate any feedback for my buddy. Thanks.

I would try a 25mcg increase in levo first. Her ft4 is not 50% through range so there is room for an increase. If ft3 is then still low it will be time to explore t3 options.

FT4: 1.2 pmol/l (Range 0.8 - 1.8)

Ft4 only 40.00% through range

She needs 25mcg dose increase in levothyroxine

Which brand of levothyroxine does she currently take

Approximately how much does she weigh in kilo

Guiding Treatment with Thyroxine:

In the majority of patients 50-100 μg thyroxine can be used as the starting dose. Alterations in dose are achieved by using 25-50 μg increments and adequacy of the new dose can be confirmed by repeat measurement of TSH after 2-3 months.

The majority of patients will be clinically euthyroid with a ‘normal’ TSH and having thyroxine replacement in the range 75-150 μg/day (1.6ug/Kg on average).

The recommended approach is to titrate thyroxine therapy against the TSH concentration whilst assessing clinical well-being. The target is a serum TSH within the reference range.

……The primary target of thyroxine replacement therapy is to make the patient feel well and to achieve a serum TSH that is within the reference range. The corresponding FT4 will be within or slightly above its reference range.

The minimum period to achieve stable concentrations after a change in dose of thyroxine is two months and thyroid function tests should not normally be requested before this period has elapsed.

She takes Synthroid and is 118k. She is so unhappy with herself and I appreciate the feedback here to get her going in the right direction.

I'm sorry k so quite a bit overweight.

118 pounds = 53 kilo x 1.6mcg = 85mcg as likely daily dose levothyroxine required

118kilo x 1.6mcg = 188mcg as likely daily dose levothyroxine

Only ever increase levothyroxine by max 25mcg at a time and retest 6-10 weeks later
